放射科X线机房的土建与防护

                    <p><span style="">    放射科是帮助医生诊断疾病的检查科室(兼做介入治疗),它的检查手段主要依赖机器完成。因此放射科是安装各种机器装置的场所(主要是</span><span style="">X</span><span style="font-size: 14px; background-color: rgb(255, 255, 255); font-family: 宋体;">线机),这些装置除了一般安置属性外还有电离辐射的特性,因此在地级以上医院大部分都建有适合这种特性的放射影像楼,而基层医院则不然。现对放射科机房的土建、防护等要求简述如下。 </span><br  /></p><p style="margin-bottom: 0;margin-top: 0;background-color: rgb(255, 255, 255)"><span style="">1 <span style="font-family:宋体">土建要求</span></span><span style=""><br  /></span><span style="">     </span><span style=""><br  /></span><span style="">  从地理位置上放射科建在远离病房和人员集中的地方,与门诊楼连体时在一层或一层一端。</span><span style="">框架结构或普通实心砖墙。实心墙体厚度<span style="font-family:Times New Roman">575px</span><span style="font-family:宋体">或</span><span style="font-family:Times New Roman">925px</span><span style="font-family:宋体">,混凝土墙体厚度</span><span style="font-family:Times New Roman">≥500px</span><span style="font-family:宋体">。</span></span><span style="">多层建筑时,楼板用混凝土灌注并有一定承重,因为大型机器机体较重,而且频繁运动产生震动,因此有些大型机器要求楼板承重<span style="font-family:Times New Roman">1000kg/m 2 </span><span style="font-family:宋体">。 </span></span></p><p style="margin-bottom: 0;margin-top: 0;background-color: rgb(255, 255, 255)"><span style="">2 <span style="font-family:宋体">水、电等要求 </span></span><span style=""><br  /></span><span style="">     </span><span style=""><br  /></span><span style="">  <span style="font-family:Times New Roman">2.1</span></span><span style=""> 水是放射科不可少的 特别是暗室内,要利用水冲洗胶片(干式胶片除外)</span><span style="">,故应保证水的上下通畅,但其他机房内一般不需自来水。</span><span style=""><br  /></span><span style="">    </span><span style=""><br  /></span><span style="">  <span style="font-family:Times New Roman">2.2 </span><span style="font-family:宋体">电是放射科所有机器的动力 </span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机属于大功率精密设备,供电优劣直接影响着机器的性能发挥。</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机对电源的要求包括</span><span style="font-family:Times New Roman">:</span><span style="font-family:宋体">电源的容量、内阻、频率、电压等。</span></span><span style=""><br  /></span><span style="">    </span><span style=""><br  /></span><span style="">  <span style="font-family:Times New Roman">2.2.1 </span><span style="font-family:宋体">电源容量 </span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机所需电源容量在</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机安装说明书中都有规定。放射科所需电源总容量要根据设计安装机器台数和每台</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机所需电源容量进行计算。安装单台</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机时按安装说明书要求即可</span><span style="font-family:Times New Roman">;</span><span style="font-family:宋体">安装多台机器时,通常可以按每台机器实际所需容量之和的</span><span style="font-family:Times New Roman">1/3</span><span style="font-family:宋体">选择,因为多台机器不可能在同一瞬间内曝光。</span></span><span style=""><br  /></span><span style="">    </span><span style=""><br  /></span><span style="">  <span style="font-family:Times New Roman">2.2.2 </span><span style="font-family:宋体">电源内阻 </span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机是瞬间大功率负载,在摄影时供电电流很大。大型</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机可达几百安培电流,所以电源内阻对电源电压的影响很大,直接影响到</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机的正常工作甚至造成机器电路元件的损坏。电源内阻是</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机设计时的重要技术参数,每台</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机都根据机器容量大小规定电源内阻。我国在</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机系列产品规范中要求,使用</span><span style="font-family:Times New Roman">380V</span><span style="font-family:宋体">电源电压</span><span style="font-family:Times New Roman">25kW</span><span style="font-family:宋体">的高压装置时电源内阻值为</span><span style="font-family:Times New Roman">0.3Ω</span><span style="font-family:宋体">,</span><span style="font-family:Times New Roman">50kW</span><span style="font-family:宋体">时</span><span style="font-family:Times New Roman">0.15Ω</span><span style="font-family:宋体">,</span><span style="font-family:Times New Roman">100kW</span><span style="font-family:宋体">时</span><span style="font-family:Times New Roman">0.1Ω</span><span style="font-family:宋体">。电源内阻是指电源变压器内阻和电源导线内阻之合(变压器内阻和绝缘导体阻值在电工手册中能查到)。当变压器选定之后电源内阻主要是电源导线电阻,它取决于电源线的材料、长度、截面积,因此应尽可能使放射科的工作场所与变压器机房距离近一些或者增加电源线导体的截面积。</span></span><span style=""><br  /></span><span style="">    </span><span style=""><br  /></span><span style="">  <span style="font-family:Times New Roman">2.2.3 </span><span style="font-family:宋体">电源电压、频率、相位 一般</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机对电源电压都有明确的要求(</span><span style="font-family:Times New Roman">220V</span><span style="font-family:宋体">或</span><span style="font-family:Times New Roman">380V</span><span style="font-family:宋体">)且电压波动范围在</span><span style="font-family:Times New Roman">-15%</span><span style="font-family:宋体">~</span><span style="font-family:Times New Roman">+10%</span><span style="font-family:宋体">。我国的供电频率为</span><span style="font-family:Times New Roman">50Hz</span><span style="font-family:宋体">,国产</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机均按</span><span style="font-family:Times New Roman">50Hz</span><span style="font-family:宋体">设计,国外</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机有的按</span><span style="font-family:Times New Roman">60Hz</span><span style="font-family:宋体">设计,但它内部有的电路可以调整为</span><span style="font-family:Times New Roman">50Hz</span><span style="font-family:宋体">电路,电源频率要求</span><span style="font-family:Times New Roman">±0.5Hz</span><span style="font-family:宋体">。电源相位要相对稳定,当改变相位时</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机三相驱动马达的转动方向也发生改变。</span></span><span style=""><br  /></span><span style="">    </span><span style=""><br  /></span><span style="">  <span style="font-family:Times New Roman">2.2.4 </span><span style="font-family:宋体">地线 为了保证工作人员和患者的安全,</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机要有良好的接地。接地的意义有两方面</span><span style="font-family:Times New Roman">:</span><span style="font-family:宋体">工作接地和保护接地。工作接地是电路工作时需要接地,如高压次级中心接地。保护接地是将</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机不带电金属外壳或金属元件外壳与接地装置进行连接,一旦绝缘破坏或击穿漏电则与大地连接,起安全保护作用。接地线用</span><span style="font-family:Times New Roman">≥4mm 2 </span><span style="font-family:宋体">的多股铜线与地极焊接牢固成为一体。接地电极一般用面积</span><span style="font-family:Times New Roman">&gt;0.25m 2 </span><span style="font-family:宋体">、厚度</span><span style="font-family:Times New Roman">&gt;3mm</span><span style="font-family:宋体">的铜板制成。埋设于离建筑物</span><span style="font-family:Times New Roman">3m</span><span style="font-family:宋体">以外地下</span><span style="font-family:Times New Roman">1.5m</span><span style="font-family:宋体">地槽内,周围放木炭和食盐,保证地电极周围湿润。自来水管作地线时(小型</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机),用宽</span><span style="font-family:Times New Roman">75px</span><span style="font-family:宋体">厚</span><span style="font-family:Times New Roman">1mm</span><span style="font-family:宋体">的铜皮紧固于自来水管上。我国规定</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机的接地阻值应</span><span style="font-family:Times New Roman">&lt;4Ω</span><span style="font-family:宋体">。</span></span></p><p style="margin-bottom: 0;margin-top: 0;background-color: rgb(255, 255, 255)"><span style="">  <span style="font-family:Times New Roman">2.2.5 </span><span style="font-family:宋体">避雷 现在设备大都使用集成电路和微机控制,对外部电源要求严格。当雷电发生引起外部电路电源改变,一个大的冲击电流进入正在开机使用的设备,可对集成电路造成毁灭性的破坏和电子元件的不可修复故障。因此不仅要在房屋建筑安装避雷装置,有条件的地方还应在机器电源配电箱上安装避雷装置,以防雷击。 </span></span></p><p style="margin-bottom: 0;margin-top: 0;background-color: rgb(255, 255, 255)"><span style="">3 <span style="font-family:宋体">房屋要求</span></span><span style=""><br  /></span><span style="">     </span><span style=""><br  /></span><span style="">  <span style="font-family:Times New Roman">3.1 </span><span style="font-family:宋体">一般要求 布局要合理,要根据用途安排房间,要有利于工作秩序和提高工作效率、方便患者,有利于</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机的安装维修并同时注意防护。</span></span><span style="">室内应干燥、防尘、防鼠<span style="font-family:Times New Roman">;</span><span style="font-family:宋体">设有空调或者暖气</span></span><span style="">。电源配电箱和穿线孔预留在合适的位置。</span><span style=""><br  /></span><span style="">    </span><span style=""><br  /></span><span style="">  <span style="font-family:Times New Roman">3.2 </span><span style="font-family:宋体">机房空间大小 应以保证工作安全方便为前提。按照国家防护标准规定</span><span style="font-family:Times New Roman">:</span><span style="font-family:宋体">机房应有足够的使用面积,新建机房,一般安装</span><span style="font-family:Times New Roman">100mA</span><span style="font-family:宋体">以下</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机</span><span style="font-family:Times New Roman">≥24m 2 </span><span style="font-family:宋体">,</span><span style="font-family:Times New Roman">200mA</span><span style="font-family:宋体">以上的可</span><span style="font-family:Times New Roman">≥36m 2 </span><span style="font-family:宋体">,多球管</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机房面积可酌情扩大</span><span style="font-family:Times New Roman">;</span><span style="font-family:宋体">机房高度由于机器结构不同要求不一,一般在</span><span style="font-family:Times New Roman">2.5</span><span style="font-family:宋体">~</span><span style="font-family:Times New Roman">3.5m</span><span style="font-family:宋体">之间。</span></span><span style=""><br  /></span><span style="">    </span><span style=""><br  /></span><span style="">  <span style="font-family:Times New Roman">3.3 </span><span style="font-family:宋体">机房其他结构 我国《医用诊断</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线防护设施标准》中规定</span><span style="font-family:Times New Roman">:</span></span><span style="">机房门、窗必须合理设置,同样要有适合铅当量的防护厚度,</span><span style="">使受检者得到相应的防护。</span><span style="">机房要保持良好的通风,安装送新风系统或换气装置。</span><span style="">因为<span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线电离空气对人体有害,机房门外要设工作指示灯。隔室操作室与机房间的观察窗有足够铅当量的铅玻璃。 </span></span></p><p><span style=""> <span style="font-family:Times New Roman">4 </span><span style="font-family:宋体">防护要求</span></span><span style=""><br  /></span><span style="">     </span><span style=""><br  /></span><span style="">  放射科工作场所与其他科室的区别,不仅是安装机器的区别,还有一个重要的区别在于<span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机发出的射线辐射。这关系到工作人员、患者和周围外环境人员的健康。因此,</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">线机房的设计要把防护放在第一位。根据我国《医用诊断</span><span style="font-family:Times New Roman">X</span><span style="font-family:宋体">射线机卫生防护标准》,</span></span><span style="">机房中射线束朝向的墙壁应有<span style="font-family:Times New Roman">2mm</span><span style="font-family:宋体">铅当量的防护厚度(主防护),其他侧墙壁和天棚(多层建筑)应有</span><span style="font-family:Times New Roman">1mm</span><span style="font-family:宋体">铅当量的防护厚度(副防护)  </span></span><span style=""><br  /></span><span style=""> </span><span style=""><br  /></span><span style="">  透视机房的墙壁均有<span style="font-family:Times New Roman">1mm</span><span style="font-family:宋体">铅当量的防护厚度。以普通砖墙为例,一般</span><span style="font-family:Times New Roman">600px</span><span style="font-family:宋体">厚的实心砖墙只要灰浆饱满不留缝隙即可达到</span><span style="font-family:Times New Roman">2mm</span><span style="font-family:宋体">铅当量,空心砖或砖缝灰浆不饱满时不能达到</span><span style="font-family:Times New Roman">2mm</span><span style="font-family:宋体">铅当量。</span></span><span style="">除墙壁防护外,门窗、通风口、穿线孔、传片箱、观察窗等都要有防护措施。</span><span style=""><br  /></span><span style="">     </span><span
