/></p><p><span>据悉，为了全面做好质子治疗临床试验前软、硬件准备工作，瑞金医院肿瘤(质子)中心已试运营，开始肿瘤的放疗、化疗、靶向治疗等肿瘤综合治疗，为年底国产质子治疗的临床试验做好准备。</span></p><p><br /></p><p><span><strong><span>国内质子治疗发展略有落后<br /></span></strong></span></p><p><br /></p><p><span>我国质子治疗起步相对较晚，尽管相比于美国和日本还有一定的差距，但总体来说，国内粒子治疗的发展并没有显著落后于世界其他国家，根据PTCOG官网统计：截至2018年7月，全球共有74家已运营的质子重离子治疗中心，其中质子治疗中心63家，重离子治疗中心5家，质子/重离子治疗中心6家。数据中未包括近日开始营业的兰州武威重离子医院和本文提到的上海交通大学医学院附属瑞金医院肿瘤(质子)中心。</span></p><p><br /></p><p><span>国内最早的粒子治疗中心，如山东万杰在运营几年后，曾因技术维护的问题无法解决（缺乏具备相应粒子放疗技术的物理师）而被迫停止运营。而在上海质子重离子医院<span>成立之初，便着手培养了大批优秀的物理师储备人才，同时引进国外专家，因此我们才能说在设备使用、运营等方面国内不输国外。</span></span></p><p><br /></p><p><span>然而在设备研发进度上，国内确实要比发达国家略逊一筹。2005年落成的山东淄博万杰医院博拉格质子治疗中心设备提供方为比利时的IBA公司；2015年正式运营的上海质子重离子医院（复旦大学附属肿瘤医院质子重离子中心）采用的是西门子的质子设备。</span></p><p><br /></p><p><span><strong><span>全球质子设备提供商<br /></span></strong></span></p><p><br /></p><p><span><span>根据公开数据显示，全球目前有11家提供质子/重离子治疗设备生产厂商。</span></span></p><p><br /></p><p><strong><span>1.ProNova</span></strong></p><p><br /></p><p><p><img src="image/20201014/3c857f930b964fc44b1f24316661a0f7_4.jpg" /></p><br /></p><p><br /></p><p><span>ProNova前身是全球最大的PET-CT设备公司CTI，在2006年以10亿美元的价格卖给西门子后，集中精力于质子治疗设备，旨在开发成本更低、技术更先进的质子治疗系统。</span></p><p><strong><span>2.Varian </span></strong><span></span></p><p><br /></p><p><p><img src="image/20201014/21805573cebe9c066651e5d2801d2165_5.png" /></p></p><p><br /><span>Varian医疗系统公司（Varian Medical Systems）创立于1948年，2014年出售第一台完全意义上的质子设备。其产品新升级的ProBeam紧凑型质子治疗系统能够实施全方位旋转笔形束扫描，能够360度向患者发射放射性、采集患者图像，为临床工作人员提供了更多的选择。同时还使用了CBCT为患者采集摆位图像，可获得高质量的解剖图像和优质的软组织显影。</span></p><p><br /></p><p><strong><span>3.IBA</span></strong><span></span></p><p><br /></p><p><p><img src="image/20201014/71901d03d0301b5b0dd902962da8ac80_6.png" /></p></p><p><br /><span>IBA（Ion Beam Applications S.A）成立于1986年，于1998年引入了粒子回旋加速器，占有质子治疗的市场份额达到50%以上，有超过25家以上的质子治疗设施是IBA系统，有超过25000名以上的患者采用IBA的设备进行治疗。其产品Proteus?ONE，作为一种小型、更经济的解决方案，可以让更多的患者得到质子治疗。</span></p><p><br /></p><p><strong><span>4.日立</span></strong></p><p><br /></p><p><p><img src="image/20201014/6b08b8c52951e4ef769b6f2601cde59a_7.jpg" /></p></p><p><br /></p><p><span>日本第一台点扫描质子治疗系统来自于日立，该系统于2013年1月7日开始治疗患者(名古屋质子治疗中心)。日立的笔形束扫描质子治疗设备是第一个获得美国FDA许可的该类型质子治疗设备，也是第一个在美国商业医院开始进行点扫描治疗的质子治疗设备。</span></p><p><strong><span>5.住友</span></strong></p><p><br /></p><p><p><img src="image/20201014/678f39d3e98dbeddba017bd7836c538c_8.png" /></p><br /></p><p><br /></p><p><span>住友公司是世界首批质子系统供应商之一，1971年以来，公司研发核物理实验和应用的各种回旋加速器，RFQ型直线加速器和小型同步光环等研制开发工作。1998年向日本柏市国立癌症中心东院交付了该公司首台商用系统，该系统用于一个230zzMeV质子回旋加速器和360度单层或多层旋转门架。</span></p><p><br /></p><p><strong><span>6.三菱电机</span></strong></p><p><br /></p><p><p><img src="image/20201014/e6d146914a81a21c2934062600412a36_9.png" /></p></p><p><br /><span>日本三菱电机公司是世界上目前建设质子重离子治疗中心和接受癌症病人质子射线治疗数量最多的机构。共建设10座质子重离子治疗中心，其中重离子中心4座。</span></p><p><br /></p><p><strong><span>7.OPTIVUS</span></strong></p><p><br /></p><p><p><img src="image/20201014/8aed7fbeaafec8f118adb345bbab9088_10.png" /></p></p><p><br /><span>美国OPTIVUS质子治疗中心股份有限公司（简称OPTIVUS），于1993年成立。1987年进入研发，1990年销售第一台质子治疗设备，1993年生产厂第一台商业化质子设备。</span></p><p><br /></p><p><strong><span>8.Mevion</span></strong><span></span></p><p><br /></p><p><p><img src="image/20201014/37874a7ab1e8cfa9b1809ca04d5077d8_11.jpg" /></p></p><p><br /><span>Mevion 医疗系统（Mevion Medical Systems）主打产品为小型、紧凑型的治疗设备。Mevion设备是目前世界上占地面积最小、价格最低廉的质子治疗设备，只需2000万—3000万美元即可。Mevion公司旗舰产品Mevion S250质子治疗平台是目前市场上唯一的模块化单间质子治疗系统。</span></p><p><strong><span>9.ProTom</span></strong><span></span></p><p><br /></p><p><p><img src="image/20201014/bfcc8a62f1163df6707903b1d72f8223_12.png" /></p></p><p><br /><span>ProTom International(简称ProTom)公司位成立于2007年，专门从事于用小型加速器开发出比常规质子治疗装置更小空间的质子治疗中心。和麻省Middleton的Bates MIT直线加速器中心的研究人员合作开发先进的质子治疗装置。</span></p><p><br /></p><p><strong><span>10.东芝</span></strong></p><p><br /></p><p><p><img src="image/20201014/71e74c39b2035cb6804ebf60fa3bad61_13.png" /></p></p><p><br /><span>东芝公司于1875年在东京成立，2014年8月新成立的东芝医疗保健公司专注于医疗保健业务，成为东芝全部业务的3大支柱之一。东芝将医疗系统公司提供的影像学诊断技术与核心业务衍生出的加速器和超导技术相结合，提供用于癌症治疗的重离子疗法系统。2015年东芝公司于IBA公司签署全球合作协议，以拓展世界各地的先进粒子疗法可及性。东芝公司成为IBA的Proteus?ONE在日本的经销商，IBA成为东芝碳疗法在日本以外的代理商。</span></p><p><br /></p><p><strong><span>11.中国科学院近代物理研究所</span></strong></p><p><br /></p><p><p><img src="image/20201014/dfc2f0e1160fa9b1eab4bdee5d4d6b96_14.png" /></p></p><p><br /><span>中国科学院近代物理研究所创建于1957年，是一个依托大科学装置，主要从事重离子物理基础和重离子束应用研究、相应发展先进粒子加速器及核技术的基地型研究所。经过半个多世纪的发展，已经成为在国际上有重要影响的重离子科学研究中心。</span></p><p><br /></p><p><span><span>瑞金医院肿瘤（质子）中心的建成，很有可能将是国内首个装配国产质子设备的质子中心，而设备</span></span><span>中科院上海应用物理研究所也将栖身于这份名单，成为全球第12家质子设备提供方。</span></p><p><br /></p><p><span><strong><span>多年筹备终见曙光<br /></span></strong></span></p><p><br /></p><p><span>瑞金医院肿瘤（质子）中心于2014年开始基本建设，中心位于嘉定新城核心区，项目总建筑面积26370平方米，集肿瘤质子临床医疗、教学科研、培训等功能于一体，该项目是我国将质子治疗从理论转到现实的第一次尝试。<strong>整个装置由上海应用物理研究所负责研发，艾普强粒子设备有限公司负责产业化以及装置的运行和维护。瑞金医院负责质子中心基本建设和配套病房、装置验收和临床试验的主体实施，以及除质子加速器治疗装置外的其他医疗设施和设备的提供。</strong></span></p><p><br /></p><p><strong><span>整个装置包括质子加速器、束流输运线、固定治疗室、旋转治疗室、眼束治疗室、实验室等。而旋转机架是该装置的另一个重要设备，在国内尚属首次自行研制，运行设计精度达到0.5毫米。</span></strong><span></span></p><p><br /></p><p><span>瑞金医院院长、中国工程院院士宁光介绍，瑞金医院肿瘤(质子)中心历经数年建设，今后将以自主研发、具备可持续创新能力的质子治疗装置为核心，依托瑞金医院和交大医学院强大的医教研实力，发挥肿瘤多学科综合诊治的团队优势，打造为国内质子治疗和肿瘤综合治疗的示范基地和创新基地，建设成为具有国际先进水平的肿瘤医学中心。</span></p><p><br /></p><p><span>国产质子设备的完成，将为国内相关研究提供巨大帮助，上海交通大学医学院院长、中科院院士陈国强提出，瑞金医院肿瘤(质子)中心将继续建设成为集医教研管和产学研用为一体的肿瘤研究、诊治基地，此外瑞金医院还将与中国科学院上海高等研究院成立“上海粒子束应用联合研究中心临床应用研究基地”，同艾普强公司合作成立“上海艾普强粒子设备有限公司质子装置研发与培训基地”、携手联影公司成立“瑞金联影肿瘤放射影像诊疗研发中心”。为国内相关人员提供更好的研究环境。</span></p><p><br /></p><p><span>此外，质子设备国产化还将为国内的肿瘤患者带来福音，质子/重离子的治疗费一直受人诟病，<span>美国一般治疗完成需要12-15万美元，约折合人民币90万元左右；</span><span>日本在300万日圆之下，相当于18万人民币左右；</span><span>德国需要5-6万欧元，约人民币40万左右；</span><span>国内价格在20-30万人民币左右。由于目前我国质子治疗尚未列入医保，因此高昂的治疗费用将会让大量患者望而却步。</span></span></p><p><br /></p><p><span><span>根据以往经验，当医疗设备实现国产化，设备成本下降后，治疗费用往往也会大幅下调，因此或许当瑞金医院肿瘤(质子)中心正式运营，国内的质子治疗费用也将迎来全面下调，惠及更多患者。</span></span></p><p><br /></p><p><span><span>来源：质子中国、知乎等 整编</span></span></p><p><br
