突发！Moderna新冠候选疫苗Ⅲ期临床试验被传延期

7月2日报道，Moderna公司正在改变试验计划，推迟预计的三期临床试验开始日期。但是具体推迟多长时间还不得而知。</span></p><p><br  /></p><p><span>消息传出后，Moderna股价周四一度下跌近10％。随后，Moderna首席执行官Stephane Bancel回应称该公司仍计划在7月进行试验。Bancel回应后，Moderna股价反弹，跌幅收窄至约5%。</span></p><p><br  /></p><p><span>在Moderna此前的试验中，主要涉及25微克、100微克与250微克的注射剂量。对于即将进行的三期试验，Moderna只选择100微克进行研究，该公司称这一中间剂量能在让免疫反应最大化的同时，让副作用最小化。</span></p><p><br  /></p><p><span>如果按照这一剂量水平，Moderna疫苗年产量可能会达到5亿支。到明年，通过在美国生产基地的生产以及与瑞士制药公司Lonza的合作，年产量甚至有可能上升至10亿支。</span></p><p><br  /></p><p><span>此前Bancel曾表示，如果一切顺利，其新冠疫苗预计将从7月开始启动最终阶段的临床试验，最快可能在感恩节之前获得其新冠病毒疫苗的有效性数据，2021年初投入市场。</span></p><p><br mRNA新冠候选疫苗一直备受关注。之前贝壳社也多次报道。它的速度有多快呢，举个可能不太恰当的例子，大概是一场1万米的马拉松，别人还刚刚跑1000米，它已经冲到5000米了。就连美国著名传染病专家安东尼福奇也曾赞许，称该疫苗在病毒测序后的62天内即开始临床研究，这一定程度创造了世界纪录。</span></p><p><br  /></p><p><span>当然这离不开包括美国国家过敏和传染病研究所（NIAID），CEPI、盖茨基金会等强大的背景支持。</span></p><p><br  /></p><p><span>然而，Moderna公布临床试验结果的方式以及缺乏有关候选疫苗疗效的关键信息，引发了不少医学专家的质疑及争议。有批评人士称，Moderna是为了盈利而过分“吹捧”其试验结果，因为在公布了临床一期结果后仅几个小时，该公司就宣布向公众出售1760万股。趁股价飙升之际，该公司高管及其主要投资者纷纷在暗中抛售手中的股票牟利的举动也令人生疑。自今年以来，Moderna股价已经暴涨了200%。甚至有反腐败组织呼吁致函SEC，呼吁调查Moderna高管的股票交易。</span></p><p><br  /></p><p><span>据《科创板日报》6月28日报道，近日生命科学预印本网站bioRxiv发布了Moderna公司mRNA新冠疫苗的临床前小鼠试验数据。论文显示，该疫苗可以诱导有效的中和抗体和CD8 T细胞反应，并且可以保护小鼠免受SARS-CoV-2感染。目前，疫苗正处于二期临床试验中，其疗效评估正朝着第三阶段发展。业内人士认为，该疫苗抗原设计优化充分，小鼠试验结果达到预期，足以支持人体临床试验的进行。</span></p><p><br  /></p><p><span>然而，今日突然传出其将修改试验的消息，具体原因还不得而知。消息传出后，Moderna股价周四一度下跌近10％。随后，Moderna首席执行官Stephane Bancel回应称该公司仍计划在7月进行试验。“我们一直说是7月。我确认是7月。”</span></p><p><br  /></p><p><span>Bancel回应后，Moderna股价反弹，跌幅收窄至约5%。不过值得玩味的是，该公司高管还在继续抛售股票，从2020年6月30日至2020年7月1日，公司董事、高管Bancel Stephane、Zaks Tal Zvi净卖出1.83万股。</span></p><p><br  /></p><section data-opacity="1" powered-by="gulangu"><p><span>根据世界卫生组织的统计，已有17个小组的COVID-19疫苗进入到人体临床试验。在新冠mRNA候选疫苗中，除了Moderna外，辉瑞公司与BioNTech近日也宣布了其合作开发的mRNA疫苗BNT162b1，I期和部分II期的早期临床试验数据公布在预印本平台Medrxiv上，这种疫苗的安全性和有效性数据都较为出色。</span></p><p><br  /></p><p><span>辉瑞公司与BioNTech还表示，计划在7月下旬开始一项大型的全球2/3期试验，目前还等待批准，试验将包括多达3万名健康参与者。</span></p><p><br  /></p><p><span>如果一切顺利，两家公司希望在今年年底前生产1亿剂疫苗，2021年生产多达12亿剂。</span></p><p><br  /></p><p><span>另外一个同样采用核酸技术的CureVac，也同样是“明星公司”。此前贝壳社曾报道过，CureVac一度被美国政府和德国政府争抢，德国政府为防止其外流上个月刚刚掏了3亿欧元的真金白银。此外，CureVac也获得了盖茨基金会和CEPI的支持。今日，特斯拉CEO埃隆马斯克也盯上了这家德国疫苗公司。埃隆马斯克在推特上表示，“特斯拉作为一个次要项目，正在为CureVac和可能的其他公司建造RNA微工厂。”</span></p><p><span> </span></p><p><span>不过，CureVac的疫苗进展相对前面两者会比较慢一些。早在6月中旬，该公司宣布已获得监管部门批准COVID-19疫苗的1期试验，并表示1期试验将招募168位18至60岁的人，参与者将接受2 μg至8 μg的CVnCoV剂量，以确定疫苗的最佳剂量并评估其安全性和免疫特性。届时，如果结果偏向积极，那么更大规模的试验将最早于9月或者10月开展。</span></p><p><br  /></p><p><span>值得一提的是6月30日，FDA刚刚发布了《行业指南：COVID-19预防性疫苗的开发及许可》的指导原则，规定了批准疫苗的标准。在指南文件中，CDER与行业交流了对COVID-19预防性疫苗在CMC（化学、生产与质量控制）、非临床数据、临床试验、上市后安全性评价等方面的一些考虑。在该指导原则中，FDA为新冠疫苗设置了一个明确的门槛：要求上市的COVID-19预防性疫苗在保护效力上要至少比安慰剂高50%。</span></p><p><span> </span></p><p><span>FDA指南还指出，除非在临床试验中“清楚地证明”了该疫苗的安全性和有效性，否则该疫苗将不被批准。如果仅在患者血流中产生抗体，则疫苗将不被批准，因为至今尚不清楚什么水平的抗体可以导致对病毒的保护。这表明公司和监管机构将密切关注试验参与者的人群研究，以确定与安慰剂组和普通人群相比，是否有更少百分比的被接种者获得了COVID-19。</span></p><p><br  /></p><p><span>此外，FDA还将要求疫苗制造商在批准后进行更多的安全监控，并且在接种疫苗后应至少一年跟踪接种疫苗的人员。</span></p><p><br  /></p><p><span>与减毒疫苗、灭活疫苗这些传统的疫苗相比，mRNA疫苗的研发特点就是一个“快”字。然而，考虑到目前三家公司采用的核酸技术，至今还没有一款疫苗获批，有部分怀疑者对于是否mRNA疫苗最终是否凑效持怀疑态度，这场盛大的疫苗研发竞赛更像是一场赌注，而全世界都在等待这场赌注的胜利。</span></p><p><br  /></p><p><span>据美国约翰斯·霍普金斯大学发布的实时统计数据显示，截止北京时间7月3日05:59，全球新冠肺炎累计确诊病例数超1094万例，目前累计确诊10,940,071例，累计死亡接近52万例，为519,852例。</span></p><p><br  /></p><p><span>不知不觉半年过去了，如果时间拉回到2019年末，谁也没想到这场新冠疫情如此来势汹汹浩浩汤汤。希望疫苗早日研发成功，全世界早日迎来自由呼吸的那一天。</span></p></section><p><br  /></p></section><section><section
