/></section><section><span>MitraClip G4建立在MitraClip的输送系统之上，为欧洲的医生提供了MitraClip首个基于夹子的技术的增强。雅培将 MitraClip G4 的钳夹数量扩大到 4 个，包括夹爪面积更大的钳夹，以扩大医生能够提供的治疗选择。</span></section><section><br /></section><section><span>MitraClip作为唯一被FDA批准应用于治疗二尖瓣病变的经导管瓣膜治疗的器械，始终是目前学术界关注的焦点。其概念起源于上个世纪80-90年代，当时正是欧美心脏外科发展的黄金时期，随着体外循环技术的成熟及对心脏疾病解剖及病理生理的深入认识，心脏瓣膜外科也有了重大的突破，其中最主要的一项突破就是法国人Carpentier教授所提出的完整的二尖瓣膜修复技术体系。</span></section><section><br /></section><section><span>由于是生理性重建二尖瓣结构且具有极佳的远期效果，Carpentier二尖瓣修复技术俨然已经成为当代心脏外科领域的金字塔尖，令人趋之若鹜。但Carpentier二尖瓣修复技术在当时（80-90年代）是高度技术依赖性外科操作（Highly Technical Demanding），且在80-90年代对于诸如前瓣/双瓣叶脱垂、Barlow病变等，其修复成功率及效果也并不理想。</span></section><section><span><br /></span></section><section><p><img src="image/20201014/01245a8a4298e1c8d361e4580027de10_4.png" /></p></section><section><br /></section><section><span>1991年，意大利学者Alfieri提出了一个被称为“Alfieri Stitch”的二尖瓣修复技术，即所谓的缘对缘修复技术（edge-to-edge repair）, 用简单、重复性高且可靠的外科技术来修复一些在当时并不适合Carpentier技术或者实施难度较大的二尖瓣病变。</span></section><section><br /></section><section><p><img src="image/20201014/cd8e1ff39a2c18415a76905f89a0ff8f_5.png" /></section><section><span>1998年，经导管介入缘对缘修复技术的概念被首次提出，最早的设计手稿显示采用导管输送缝线的方法。经过不断的工程学优化，最终呈现出商业化大规模生产的第一代MitraClip的型态。初期的MitraClip在“如何稳定瓣叶”和“如何捕获瓣叶”两大问题上经历了诸多尝试和修改。</span></section><section><br /></section><section><span>2002年，初代MitraClip原型产品问世。其设计核心是采用了可Clip Arm+Gripper的设计，Clip Arm可以做0-120°左右的运动进行瓣叶抓捕，Gripper为带有倒刺的金属丝，用以稳定瓣叶；第二代MitraClip NT增加的Gripper的展开角度从85°增加到120°；第三代进化出两个型号（XTR/NTR），以适应不同类型的瓣叶条件。</span></section><section><br /></section><section data-support="96编辑器" /></section><section><span>MitraClip自2013年起在美国市场上销售，2008年以来已在欧洲开始销售。2020年6月，MitraClip获得NMPA审批，授权在中国大陆上市，而MitraClip也成为目前全球唯一一款获得FDA、CE和NMPA注册的二尖瓣修复产品。对于许多人患有MR但可能没有可用的治疗选择的亚太地区，这些里程碑至关重要。</span></section><section><br /></section><section><span>该产品由两部分组成：二尖瓣夹及输送导管和可操控导引导管。其中，二尖瓣夹及输送导管包括三个主要组件：输送导管、可扭控套管和二尖瓣夹。该产品适用于经皮方式降低二尖瓣原发性异常[退行性二尖瓣返流]导致的显著症状性二尖瓣返流（MR≥3+）病例，经心脏团队确诊，存在二尖瓣手术高风险的患者。</span></section><section><span><br /></span></section><section><span>该产品可在避免开胸、体外循环以及心脏停搏的情况下，通过瓣叶组织对接经皮重建二尖瓣。优化国内现有二尖瓣疾病治疗手段，为手术高风险的中重度二尖瓣返流患者提供了一种可替代选择的微创治疗手段，提高其存活率并改善患者的生活质量，从而更好地满足临床需求。</span></section><section><span><br /></span></section><section><span>根据美国梅奥医学中心一项基于全人群的调查发现，各类瓣膜疾病的发病率均随年龄增长而升高，其中二尖瓣反流发病率远高于主动脉瓣狭窄、主动脉瓣反流、二尖瓣狭窄等疾病。在65至74岁人群中MR发病率为6.4%，在75岁以上人群中高达9.3%。</span></section><section><span><br /></span></section><section><span>国家心血管病中心阜外医院一项在35岁以上人群中的调查也发现MR发病率与年龄正相关，在65岁以上人群中，轻度MR发病率高达23%，中重度MR发病率达到2.3%。老龄化时代的到来使得65岁以上人口比重持续增长，在中国65岁以上人口的比重从2000年的6.91%增长到了2018年的11.19%，预计到2035年将高达17.22%。彼时中国65岁以上人口将超过2.5亿，按照中度以上MR发病率10%计算，则MR患者总量将超过2500万。</span></section><section><span><br
