AGFA CR系统影像伪影及解决对策

style="font-family:宋体">的优点随之显现,同时影像质量也越来越受到放射技师的关注。我们认为,伪影是影响</span><span style="font-family:Tahoma">CR</span><span style="font-family:宋体">影像质量的最大因素。所以我们必须充分认识伪影、分析伪影,并且拿出一套行之有效的改进方法。</span></span><span style=""><br  /></span><span style=""><br  /></span><span style="">材料与方法:从收集到的<span style="font-family:Tahoma">2003</span><span style="font-family:宋体">年</span><span style="font-family:Tahoma">6</span><span style="font-family:宋体">月至</span><span style="font-family:Tahoma">2004</span><span style="font-family:宋体">年</span><span style="font-family:Tahoma">2</span><span style="font-family:宋体">月中具有伪影的</span><span style="font-family:Tahoma">CR</span><span style="font-family:宋体">影像</span><span style="font-family:Tahoma">117</span><span style="font-family:宋体">例。均采用</span><span style="font-family:Tahoma">AGFA IP</span><span style="font-family:宋体">板,经</span><span style="font-family:Tahoma">ADC COMPACT PLUS</span><span style="font-family:宋体">和</span><span style="font-family:Tahoma">SOLO</span><span style="font-family:宋体">数字化扫描器(以下简称数描器)扫描。然后采用 </span><span style="font-family:Tahoma">AGFA LR5200</span><span style="font-family:宋体">激光打印一体化机硬拷贝,同时在</span><span style="font-family:Tahoma">GRAD</span><span style="font-family:宋体">工作站进行光盘刻录存储。摄片系统采用岛津</span><span style="font-family:Tahoma">UD150B-30 800MA</span><span style="font-family:宋体">高频</span><span style="font-family:Tahoma">X</span><span style="font-family:宋体">线机。胸片架与摄影平床滤线栅采用活动式,栅比</span><span style="font-family:Tahoma">12</span><span style="font-family:宋体">:</span><span style="font-family:Tahoma">1</span><span style="font-family:宋体">。</span></span><span style=""><br  /></span><span style=""><br  /></span><span style="">结果:排除外来因素的影响,分析各类<span style="font-family:Tahoma">CR</span><span style="font-family:宋体">影像伪影的特征及出现频率。全组</span><span style="font-family:Tahoma">117</span><span style="font-family:宋体">例中,前后次影像重叠伪影</span><span style="font-family:Tahoma">2</span><span style="font-family:宋体">例、</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板龟裂伪影</span><span style="font-family:Tahoma">2</span><span style="font-family:宋体">例、</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板扫描中卡片引起的伪影</span><span style="font-family:Tahoma">6</span><span style="font-family:宋体">例、黑条状伪影</span><span style="font-family:Tahoma">8</span><span style="font-family:宋体">例、黑块状伪影</span><span style="font-family:Tahoma">17</span><span style="font-family:宋体">例、</span><span style="font-family:Tahoma">PMT</span><span style="font-family:宋体">(光导器)灰尘伪影</span><span style="font-family:Tahoma">1</span><span style="font-family:宋体">:前后次影像重叠伪影(除外操作上重复失误):前后两次影像同时出现在一幅图像上。仔细观察后发现,前一次影像整体密度明显低于后一次,对诊断造成较大影响。出现此类伪影与单位时间内强光消隐灯功率、亮度有关。由于 </span><span style="font-family:Tahoma">AGFA CR</span><span style="font-family:宋体">数描器内的强光消隐灯数为 </span><span style="font-family:Tahoma">10</span><span style="font-family:宋体">枚,每枚的功率为</span><span style="font-family:Tahoma">100W</span><span style="font-family:宋体">,总功率是</span><span style="font-family:Tahoma">1KW</span><span style="font-family:宋体">。随着 </span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板流量增大,强光灯长时间连续工作,发光性能会下降,造成残留影像加剧。另外,外部供电电压不稳定、消隐灯电源控制板故障及灯座接触不良同样可造成前次影像残留现象加剧。</span></span><span style=""><br  /></span><span style=""><br  /></span><span style="">2<span style="font-family:宋体">:</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板龟裂伪影:</span><span style="font-family:Tahoma">AGFA CR</span><span style="font-family:宋体">系统数描器采用弧形通道,而</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板是具有柔韧性的软板,吸盘反复吸附作用和通过弯道时互相摩擦,易造成龟裂纹产生。开始出现在</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板边缘,影像上表现为簇状分布的细线状高密度条纹,以后将向板的中央部分延伸。在严重影响诊断质量时,必须更换</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板。</span></span><span style=""><br  /></span><span style=""><br  /></span><span style="">3<span style="font-family:宋体">:</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板扫描中卡片引起的伪影:线路停电、</span><span style="font-family:Tahoma">PMT</span><span style="font-family:宋体">(光导器)系统侧面导向板角度变化及数描器整机水平度失准等均易引起</span><span style="font-family:Tahoma">IP</span></span><span style=""> </span><span style="">Jam<span style="font-family:宋体">(卡片)故障。影响正常扫描秩序,并中断扫描过程。位于左侧出片门下方的</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板往往一部分停留于</span><span style="font-family:Tahoma">PMT</span><span style="font-family:宋体">系统内。此时将</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板取出,放回暗盒内,重新打号再扫描。可发现影像整体密度分布不均,图像类似被切割过,其间可显示明显分界带,与</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板边缘平行。若除去以上不良因素影响,重启数描器,一般都能防止该伪影发生。</span></span><span style=""><br  /></span><span style=""><br  /></span><span style="">4<span style="font-family:宋体">:黑条状伪影:在投照胸椎、腰椎侧位时,常有数条与身体长轴平行的黑条状伪影,宽约</span><span style="font-family:Tahoma">2</span><span style="font-family:Times New Roman">—</span><span style="font-family:Tahoma">3MM</span><span style="font-family:宋体">。一般与椎体部位重叠,从上方贯穿于影像下方。这可能是所摄部位的密度与相应直接空气嚗光区密度相差悬殊所造成的。一般采用缩小照射野或应用与照射部位相匹配的</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板即可减少上述伪影。</span></span><span style=""><br  /></span><span style=""><br  /></span><span style="">5<span style="font-family:宋体">:黑块状伪影:这些伪影可能是由束光计算失准造成的。通常出现于骨科影像、口腔影像,另外照射野、铅橡皮使用遮避不当也可产生。数描器具有自动设定每幅影像敏感性范围的机制,对于嚗光过度与不足的均具有纠正、调整功能。因为骨科病人肢体的金属内固定和石膏,与周围空气嚗光区形成强烈反差,很容易被软件错误计算,嚗光量小的区域会被视为无效嚗光区,用类似</span><span style="font-family:Times New Roman">“</span><span style="font-family:宋体">黑色蒙板</span><span style="font-family:Times New Roman">”</span><span style="font-family:宋体">的块状影遮盖。可以在</span><span style="font-family:Tahoma">ADC- QS</span><span style="font-family:宋体">软件工具栏黑边框软件中</span><span style="font-family:Times New Roman">“</span><span style="font-family:宋体">打开束 光</span><span style="font-family:Times New Roman">”</span><span style="font-family:宋体">功能处进行点击,即可消除。</span></span><span style=""><br  /></span><span style=""><br  /></span><span style="">6<span style="font-family:宋体">:光导器灰尘伪影:</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板表面晶体发生光致发光现象,产生荧光,被光导器(实质为光导纤维)采集并馈入光电倍增管。光电倍增管输出电压的大小与光导器输入光线的强弱密切相关。当光导器内有</span></span><span style="">灰尘聚集</span><span style="">时,光电倍增管局部信号转换能力减弱,输出电压下降,在<span style="font-family:Tahoma">CR</span><span style="font-family:宋体">影像上会</span></span><span style="">残留细线状白色伪影</span><span style="">,一般只出现一条且通常位于边缘处,其分布位置与<span style="font-family:Tahoma">IP</span><span style="font-family:宋体">暗盒扫描放置方向有关。解决方法是:扫描室保持清洁、数描器</span><span style="font-family:Tahoma">PMT</span><span style="font-family:宋体">系统处附带有钢丝绳毛刷,反复转动、抽动即可消除上述伪影。</span></span><span style=""><br  /></span><span style=""><br  /></span><span style="">7<span style="font-family:宋体">:</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板及暗盒灰尘伪影:通常我们会碰到</span><span style="font-family:Tahoma">CR</span><span style="font-family:宋体">影像上出现小点状和小片状高密度影,这些都是</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板上的灰尘污点及暗盒表面的致密污物(如石膏)造成的。所以必须定期对</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板和暗盒表面进行清理,</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板擦拭应用柔软的棉布,在无专用清洁剂的情况下,可采用高纯度无水酒精代替,在擦拭过程中,应保证双手洁净,以免在</span><span style="font-family:Tahoma">IP</span><span style="font-family:宋体">板上残留指纹影。</span></span><span style=""><br  /></span><span style=""><br  /></span><span style="">8<span style="font-family:宋体">:摄影条件过高产生的伪影:经过实践证明,</span><span style="font-family:Tahoma">CR</span><span style="font-family:宋体">系统摄影条件中</span><span style="font-family:Tahoma">MAS</span><span style="font-family:宋体">较绿屏</span><span style="font-family:Times New Roman">—</span><span style="font-family:宋体">绿片组合高</span><span style="font-family:Tahoma">20%-30%</span><span style="font-family:宋体">,才能使影像分布的量子噪声减小到可接受程度,这样嚗光指数</span><span style="font-family:Tahoma">Lgm</span><span style="font-family:宋体">可以控制在</span></span><span style="">1.8-2.2</span><span style="">之间。然而一味地追求较高摄影条件,不关注所摄病人的病理状态,必定会造成伪影的产生。如:慢支肺气肿急性发作病人,多数表现为呼气性呼吸困难并伴气促、咳嗽症状,常常摄胸片以观察是否合并气胸、纵隔气肿以及因长期咳嗽、咯痰而引起的疲劳性肋骨骨折。这类病人无论<span style="font-family:Tahoma">KVP</span><span style="font-family:宋体">、 </span><span style="font-family:Tahoma">MAS</span><span style="font-family:宋体">均比常人低。使用过高条件将超过</span><span style="font-family:Tahoma">CR</span><span style="font-family:宋体">数描器本身对影像密度、对比度的调节范围,无法遵循</span><span style="font-family:Tahoma">CR</span><span style="font-family:宋体">影像运行原理四象论进行工作。嚗光直方图显示直方图分布范围明显变窄,整体向右侧平移较明显。影像灰雾度加大,两肺野过度黑化,似泼墨状分布,无法观察到肺纹理及气胸带,上部肋骨皮质边缘似虫噬样缺失,肋骨小梁模糊无法观察,达不到诊断和技术质控要求,因此必须对</span><span style="font-family:Tahoma">CR</span><span style="font-family:宋体">摄影条件进行规范。</span></span><span style=""><br  /></span><span style=""><br  /></span><span style="">9<span style="font-family:宋体">:后处理不当产生的伪影:</span><span style="font-family:Tahoma">AGFA CR</span><span style="font-family:宋体">系统</span><span style="font-family:Tahoma">MUSICA</span><span style="font-family:宋体">后处理软件为我们日常工作提供了便捷,但不理解其各项参数含义,盲目调节会对图像质量产生影响。如在长骨影像调节中,边缘增强应谨慎使用,过度增强会在长骨的边缘出现一条波浪状的白线影,很容易误诊为轻度骨膜反应,影响了对病灶的观察。其实我们在使用</span><span style="font-family:Tahoma">CR</span><span style="font-family:宋体">的初期,就需要对后处理参数不同所得的各类影像进行对比观察,倾听科室内各方人员建议,最后将参数范围固定,以提高影像质量。以上列举的伪影类型,是我们在日常工作中所碰到的。(</span><span style="font-family:Tahoma">2</span><span
