重磅医改试点启动，要革几百万药械经销商的命！

<img data-type="gif" src="http://mmbiz.qpic.cn/mmbiz_gif/dx3KibwtDhvc6YBndBzrYvfAVPCGNAlsqB9f7SjRnXkibcO3P2CSjz7TnOO8RAArOGOjobic1D5DCTCzzAoxv9jfw/0?wx_fmt=gif" data-ratio="0.1703125" data-w="640" />6月2日，国家卫计委在深圳召开DRG收付费改革试点启动会，宣布三个城市的公立医院和3个省市级医院同步开展DRG试点。 　　在经过大量的筹划和准备工作之后，DRG收付费改革试点工作终于正式启动了。 　　DRG，医保支付改革的最终形式？ 　　作为一种创新的收付费机制，DRG收付费虽然已经在40多个国家进行了运用，对中国仍然是一个新鲜事物。 　　去年，中办、国办联合下发《国务院深医药卫生体制改革领导小组关于进一步推广深化医药卫生体制改革经验的若干意见》，其中有提出，要全面推进支付方式改革，包括：逐步减少按项目付费，推行以按病种付费为主，按人头付费、按床日付费、总额预付等多种付费方式相结合的复合型付费方式，鼓励实行按疾病诊断相关分组付费（DRGs）方式。 　　去年底，在北京召开的一次学术年会上，国家卫计委副主任王培安又透露，“十三五”期间会重点推进DRG收付费改革。 　　DRG是一项国家必然要推动的改革，而且与270个医改试点城市今年内必须推进的按病种付费改革相比，DRG是更加精细化、更具科学性的支付方式，医生的考核机制也更为完善。 　　中国卫生经济学会副会长兼秘书长、卫生服务成本与价格专业委员会主任委员、《全国按疾病诊断相关分组（DRG）收付费规范》研究组组长张振忠就表示，未来医保收付费改革的发展方向，一定是会从当前的按项目付费为主，逐步过渡到按床日、人头、病种付费，最终过渡到住院病人按DRG的多元复合型的医疗支付方式。（此处引自“健康界”） 　　就目前而言，DRG或才是可预见的我国推进医保支付改革的最终形式。 　　改革时间表： 　　 “3+3”试点先行，2020年扩大到100个城市 　　对于推进DRG收付费改革，国家相关部门早已划定路径图：全国统一版本，统一收付费规范，试点先行，以点带面，逐步扩大覆盖范围。 　　6月2日，国家卫计委在深圳市召开DRG收付费改革试点启动会，宣布3个城市和3家医院先行试点，包括：福建三明市、广东深圳市、新疆克拉玛依市，以及福建省医科大学附属协和医院、福州市第一医院、厦门市第一医院。3个城市的公立医院和3个省市级医院同步开展试点，形成“3+3”试点格局。 　　另外，目前已知的推进时间规划还有，到2018年底先行试点城市作出初步评价，2019年试点扩大到50个城市，2020年扩大到100个城市。 　　 “DRG要革经销商的命”！ 　　我国医院传统的按项目付费，是完全粗放式的，彻底刺激医生多开药、多用耗材、多做检查，从而也带动了各色代理商、经销商的无限“繁荣”，这也是为什么规模不到5000亿的医疗器械市场，获证的经营企业数量竟然达到33万家之巨的重要原因。 　　按病种付费，是相对有约束力的方式，因为医保为每一个病种支付的费用是固定的，医院、医生为了有收益，只能靠省钱。而省钱最主要办法就是减少药品、耗材的使用，减少检查。目前这个付费方式改革正在蔓延，国家要求是270个医改试点城市今年年底前要按病种收费的病种不少于100个，就已经让药械企业感到巨大压力。当漫延到所有省区、所有公立医院时，估计大量械企就已经惨遭出局了。 　　DRG付费，面向医院和医生的考核机制更为完善，对药品和医疗器械企业的冲击也更强。 　　据悉，这一次的DRG改革试点，将尝试把DRG做成一个由“医院端”到“病人端”的整体收付费方案，形成收付费两端闭环。而非“医院收费实行DRG，病人缴费仍然按项目付费”，药品和耗材仍然是医院的利益中心，DRG则沦为管理者和被管理者之间低层次的博弈。 　　只有两端均形成闭环，才能把药品和耗材真正内化为医疗机构的成本要素，调动医院和医生有效降低成本的积极性，控制医药费用不合理增长。 　　DRG要带来的是医保、医生和患者的三方共赢。医生收入更高，不靠多开药、多用耗材、多做检查来赚钱，回扣也将大大减少。医保支出可预期、可控制、减少浪费。 　　唯一、也是最大的“输家”则是药品和医疗器械企业们！ 　　《全国按疾病诊断相关分组（DRG）收付费规范》研究组组长张振忠就直言，2015年我国医生总数只有280万人，而医疗领域的经销商就有300万人，“DRG要革的是这些经销商的命。”（此处引自“健康界”） 　　DRG改革试点已经启动，几年内还要扩大到全国100个城市，甚至最终很可能要覆盖全国所有医疗机构。改革是渐进式，而不是暴风骤雨式，所以现阶段你可能感觉不到。但如果不重视并筹谋战略应对，那未来出局的可能就是你。 <p style="max-width: 100%;
