专访北大医院宫颈疾病诊疗中心主任赵健： 细说R-way阴道镜智能评估系统的由来

/></p></p><p><br  /></p><p><span>北京大学第一医院（简称“北大医院”）是一所融医疗、教学、科研、预防为一体的大型综合性三级甲等医院，是中央保健基地医院。北大医院创建于1915年，是我国最早创办的国立医院，也是国内首批建立的临床医学院之一。近百年来，北大医院拥有着一批国内的首创专业学科，以及在我国率先开展的诊疗技术，如国内首创小儿科(1940）、泌尿外科(1946)、肾脏病专业(1950)、综合医院心血管病房(50年代)、小儿神经专业(50年代)。</span></p></section></section></section><section data-role="paragraph"><p><br  /></p><p><span>北大医院多个科室处于国内领先地位，其宫颈癌防治历史可以追溯到16年前，2003年成立的北大医院宫颈疾病诊疗中心，通过多年的努力，在业内已经建立了良好的口碑，该中心接手的宫颈病变的复发率极低。能够取得如今的成绩，离不开诊疗中心的专业团队和专业设备，比如诊疗中心阴道镜室目前使用的理邦电子阴道镜，不仅图像清晰度高，利于捕捉宫颈高级别病变，软件功能贴合临床需求，而且阴道镜软件中的R-way?阴道镜智能评估系统获得了国家专利，能够协助医生快速排查高级别病变。</span></p><p><br  /></p></section></section><p><span>近日，走访北大医院，采访到医院妇产科主任医师赵健，揭开北大医院宫颈疾病诊疗中心的神秘面纱，和大家一起来了解R-way?阴道镜智能评估系统。</span></p><p><br  /></p><section 病毒研究和细胞学病理诊断领域，在攻读博士和初到北大医院期间，初步掌握了HPV检测以及宫颈细胞学检测这两大宫颈癌筛查的技术手段；而后在进行全国巡讲的过程中发现，</span><strong><span>基层医生对阴道镜诊断技术的提升需求极大，但却普遍缺乏相关的专业知识。</span></strong><span>而作为宫颈癌及</span><span>癌前病变诊断的重要环节，阴道镜可以观察发现肉</span><span>眼看不到的微小病变，进行定位并活检，降低细胞学检查的假阴性和漏诊几率。</span><span>为了改变这一现状，给基层提供更好的应用规范和提升阴道镜的操作和诊断技能，赵健主任开始专注于研究阴道镜的检查和质控。</span></p><p><span><br  /></span></p><p><span>通过对北大医院2003-2012年整体数据库进行总结，赵健主任团队结合海量患者的TCT、HPV、阴道镜图像及病理结果等数据进行分析，根据病变部位细胞增生、血管再生与病理发病机制的相关关系，总结出客观规律，进而研发出了<span>R-way?</span>阴道镜智能评估系统，并获得国家专利。</span><br  /></p><p><span><br  /></span></p></section></section><p><span>赵健主任指出，这套系统兼具科学性、先进性、实用性以及可重复性，不仅容易掌握，而且敏感度很高，对阴道镜图像的边界、醋白的厚度、血管的形态、分布以及碘反应进行评定，通过颜色视觉的配比，更容易找出病变部位。</span></p><p><span><br  /></span></p><section data-brushtype="text"><strong>R-way?实现“傻瓜式”诊断</strong></p></section></section><section></section></section></section><section data-role="paragraph" data-color="#27939d"><p><br  /></p></section></section><p><span>R-way?阴道镜评估体系可以指导医生寻找宫颈病变发生发展过程中异常细胞和异常血管的证据，用简单的方法快速找到并治疗宫颈高级别病变，从而彻底阻断宫颈癌的发展。其特点追溯于</span><span><strong>宫颈疾病的两大发病机制：</strong><strong>血管再生，细胞增生</strong></span><span><strong>。</strong></span><span></span></p><p><br  /></p><p><span>正常情况下，宫颈上皮层内没有血管，宫颈病变时由于有异常血管增生，病变部位常常显示为红色R（red），即R区；W（white）代表病变在5%醋酸作用2分钟或2分半钟后异常增生细胞表现为白色；Y（yellow）则代表碘染色后呈现黄色。</span></p><p><br  /></p><p><p><img src="image/20201014/d421e2be79011197c7b44e375fb64c31_3.png" /></p></p><p><span>理邦C6 HD 电子阴道镜</span></p><p><br  /></p><p><span>赵健主任通过搭载了R-way?阴道镜智能评估系统的理邦C6 HD 电子阴道镜展开研究。设备在均匀稳定的光源下真实还原了组织颜色，图像清晰、真实，为分辨细小病灶提供了有力保障。当阴道镜上呈现红色时代表血管再生，直观而且联系性强，而白色则代表细胞增生，故结合其图像特征将之称为R-way?。赵健主任戏称之为<strong>“傻瓜式”系统</strong>，将阴道镜图像按时间顺序同屏显示，动态对比病变部位的醋白变化和碘染色，<strong>只通过颜色视觉的配比就可以找到高级别病变部位，</strong>极为简单易学。</span></p><p><br  /></p><p><p><img /></p></p><p><br  /></p><p><span>而在此之前，由于在同一图像中要同时反映血管和细胞这两大特征，所以</span><strong><span>丰富经验的阴道镜医生才能对阴道镜诊疗进行把控，</span></strong><span>对大部分基础薄弱、临床经验匮乏的医生来说，很难发现微小的病灶。</span></p><p><br  /></p><section data-brushtype="text"><strong>传承继续，助力精准诊疗</strong></p></section></section><section></section></section></section><section data-role="paragraph" data-color="#27939d"><p><br  /></p></section></section><p><span>赵健主任认为要坚持不懈地打造好宫颈疾病诊疗中心。基于患者利益考虑，改变过去盲目的活检方式，在患者的宫颈上实现精准的诊断，即哪里有高级别病变就在哪里取活检，从而在宫颈病变的治疗上，极大程度地减小手术对患者的伤害。</span></p><p><span>&nbsp;</span></p><p><span>自2014 年起，北大医院设立了国家级继续教育项目《阴道镜实用技术培训及宫颈病变规范化临床处理培训班》，普及电子阴道镜检查技术，迄今为止已经开展将近 50 期培训班。赵健主任表示，通过阴道镜相关知识的规范化培训，阴道镜医生尤其是云南、贵州和四川等边远地区医生的阴道镜诊断技能有大幅度提高，意义重大。</span></p><p><span><br  /></span></p><section /></p></p><p><span>赵健&nbsp;博士（后）&nbsp;主任医师，北京大学第一医院宫颈诊疗中心负责人，国家级继续教育项目《阴道镜实用技术培训及宫颈病变规范化临床处理培训班》、《宫颈细胞学阅片技能培训班》项目负责人，《中国妇女宫颈健康保障工程》项目负责人。</span></p><p><br  /></p><p><span>2000年开始，历时3年构建含有HPV16 E6E7的真核表达载体，感染原代胚胎宫颈细胞，诱导宫颈细胞永生化，细胞传20代后性状稳定。</span></p><p><br  /></p><p><span>2003年博士后出站，潜心研究宫颈癌及癌前病变的诊断方式，提出宫颈癌筛查流程，包括裸眼识诊、镜下拟诊以及借助细胞学异常和出血，该流程利用宫颈疾病发病机制对临床不同阶段浅表结构的识别，能够实现宫颈癌筛查诊断的最终目的。</span></p><p><br  /></p><p><span>2013年总结过去十年的数据提出R-way</span><span></span><span>阴道镜诊断</span><span>评估系统，发表相关</span><span>文章数篇， 2016年TTR-way<span></span>培训软件获得计算机软件著作权。</span><span>目前是中国医药教育协会妇科专委会宫颈病变分会主任委员、中国医疗保健国际促进会常委、CSCCP及宫颈疾病与细胞学学院委员，中国实用妇科与产科杂志编委&nbsp;。</span></p><p><br  /></p><p><span>专业特长：宫颈脱落细胞学诊断；人类乳头瘤病毒的检测；阴道镜检查；宫颈物理治疗与LEEP手术。擅长宫颈病变精准诊断与精准治疗。</span><span></span></p><p><br  /></p></section></section></section></section><p><br  /><p><img
