/></section><section><span>几个月来，从抢口罩、囤口罩到习惯了佩戴口罩出门，口罩已经成为了生活必需品，甚至还被开玩笑称为“硬通货”。除了新冠病毒疾病大流行，及其对口罩制造业的影响以外，背后还有一个人们习以为常、容易忽视的原因：<strong>口罩是“一次性”的。</strong></span></section><section><br  /></section><section><span>回顾口罩在医学应用上的历史，20世纪60年代以来，一次性口罩逐渐取代了可重复使用的口罩。在《柳叶刀》近日发表的一篇文章中，来自瑞士日内瓦大学和加拿大麦吉尔大学的研究人员详述了这一转变的过程。结合这篇文章，我们简要回顾医用口罩的历史发展和演变。</span></section><section><br powered-by="gulangu"><section><section><p><span><strong><strong>口罩<span>的诞生：从防疫面具到预防手术感染</span></strong></strong></span></p></section></section></section></section></section></section><section /></section><section><span>在早期的现代欧洲，预防传染性疾病的卫生措施中已经包括了遮盖口鼻，主要通过在面具中塞满香料、药材以消除所谓的有毒空气。比如在“黑死病”鼠疫流行期间，医生在行医时不但会从头遮到脚，还会戴上鸟喙状的面具。</span></section><section><br  /></section><p><p><img /></section><section><span>不过，到了18世纪，这种做法已经被逐渐淘汰。而如今我们看到的医用口罩的起源，主要可以追溯到手术领域对感染因素的认识。外科口罩的诞生，起初是为了保护手术患者，防止外科医生污染他们的伤口。</span></section><section><br  /></section><section><span>1860年代，英国外科医生约瑟夫·李斯特（Joseph Lister）注意到了患者在术后伤口化脓的过程，联想到路易斯·巴斯德（Louis Pasteur）著名的肉汤实验，李斯特猜测是细菌引起了伤口感染，并在临床实践中验证了抗菌剂可以避免感染。</span></section><section><br  /></section><section><span>1880年代，又有外科医生提出无菌操作，这相当于关口前移，在源头就预防感染。这显然对手术环境提出了卫生挑战，包括医生的手部、仪器乃至呼吸都会对患者造成感染风险。当时的布雷斯劳大学（University of Breslau，现为波兰Wroclaw大学）外科主任 Johann Mikulicz和合作团队通过实验证明，呼吸道飞沫能够携带活细菌。</span></section><section><br  /></section><section><span>基于这些发现，1897年，Mikulicz医生开始佩戴口罩。在他的描述中，口罩是“用两根绳子将一块纱布绑在帽子上，覆盖脸部以遮住鼻子、嘴和胡须”。在法国巴黎，外科医生也于同年开始在手术室佩戴口罩。</span></section><section><br  /></section><section><span>自此，口罩代表了一种控制感染的新策略：预防接触细菌，而非用化学物质杀死细菌。像很多新事物一样，这种策略起初不乏争议，但很快口罩就在外科领域变得越来越普遍。后来，更多医务人员也开始习惯佩戴外科口罩，以免被工作中所接触到的患者感染。</span></section><p><br  /></p><p><p><img /></section><section><span>有意思的是，现代医学口罩虽然诞生于外科领域，不久后又和“鸟喙面具”殊途同归，在传染病防治中进一步被广泛使用，<strong>尤其是在1910-1911年中国满洲里鼠疫和1918-1919 年流感大流行期间。</strong></span></section><section><br  /></section><section><span>在满洲里，伍连德博士采取的重要防疫措施，其一是隔离患者，其二便是设计了“伍氏口罩”并大力推广。大洋彼岸，流感大流行期间，美国一些城市强制要求警察、医务工作者甚至居民佩戴口罩，在旧金山，流感死亡率下降可部分归因于口罩策略。</span></section><section><br  /></section><section><strong><span>显然，这时候，口罩的应用已经超出了手术室的范围：还能保护佩戴者免受感染。</span></strong></section><section><br  /></section><section><section powered-by="gulangu"><section><section><section powered-by="gulangu"><section><section><p><span><strong><strong><span>改良设计，关注防感染效果和长期使用</span></strong></strong></span></p></section></section></section></section></section></section><section /></section><section><span>与此同时，医用口罩也在不断发展创新。尽管口罩的功能已经得到普遍、一致的认可，但在20世纪前期，医学人员们仍在不断改良口罩的材质和设计，提高使用效率。</span></section><section><br  /></section><section><span>当时，口罩通常是由几层纱布制成的，有时还加上一层防水材料并用金属框架固定。正如诞生之初的设计，佩戴口罩的主要目的是防止呼吸道飞沫传播。而且，大多数口罩都是可清洗的，金属部件也可以消毒，从而方便口罩长期使用。</span></section><section><br  /></section><section><span>1918年曾有医学研究人员记录：“口罩可能会被反复清洗，并无限期地使用。”</span></section><section><br  /></section><section><span>为了确保使用效果，自然也有不少研究人员测试并比较了可重复使用口罩的过滤效率，方法多种多样，包括用口罩过滤细菌喷雾、或志愿者佩戴口罩后收集飞沫进行培养实验，以及在实际临床环境中开展观察性研究。<strong>总的来说，不同口罩过滤细菌的效果差别很大。但医学人员认为，当正确佩戴时，一些口罩能够防止感染</strong>。</span></section><section><br /></section><section><strong><span>20世纪30年代，一次性口罩开始出现，到60年代则逐渐变得流行。</span></strong><span>当时，护理和外科领域的专业期刊上开始出现新型过滤口罩的广告，这些口罩由无纺布合成纤维制成，均为一次性使用。</span></section><section><br  /></section><section><span>广告凸显了新型口罩的功效、舒适和方便。不同于传统医用口罩，这些口罩紧贴面部，不仅能过滤呼出的空气，也能更好地过滤吸入的空气，而针对这些功效设计的面料材质在消毒过程中容易变质，因此只能一次性使用。</span></section><section><br  /></section><section><span>1969年，随着医院内开始采纳“一次性使用系统”的观念，<strong>随着注射器、针头、托盘和手术器械等的迭代，一次性口罩也更广泛地进入了医疗系统</strong>。一方面，一次性可以改善污染风险；另一方面，也有助于减少人力成本、便于院内用品管理。何况，此前的推广效应也已经让不少医疗工作者对一次性用品持欢迎态度，比起以前要拆开封条、高压灭菌再重新组装，这确实太方便了。</span></section><section><span><br /></section><section><strong><span>那么，从功效上说，一次性是否真的比重复使用的防菌效果更优呢？研究结果并不完全一致。</span></strong><span></span></section><section><br  /></section><section><span>1975年一项关注工业化生产的可重复使用口罩的研究发现，如果设计足够好，由四层棉布制成的可重复使用口罩优于流行的一次性纸质口罩和合成材料口罩。然而，旧去新来，更多研究没有纳入传统可重复使用口罩；一些研究将手工或自制（而非工业化生产）的棉布可重复使用口罩与工业化生产的一次性口罩相比，后者细菌过滤效果更胜一筹。</span></section><section><br  /></section><section><strong><span>不过，这些并不完全全面的研究，在一定程度上提示了传统可重复使用口罩可能不安全，也在一定程度上影响了对传统口罩的进一步改良研究。</span></strong></section><section><br  /></section><section><section /></section><p><span>在新冠病毒疾病流行期间，由于前期口罩紧缺，网上不乏流传五花八门的一次性口罩“消毒”方法，也有一些科研学术机构探索消毒方法的正式研究。应对个人防护设备（PPE）短缺危机，美国FDA此前也授予了创新PPE消毒技术紧急使用授权，Battelle公司每台CCDS重症监护消毒系统每天能够对8万个N95口罩进行消毒。这一消毒系统可以对一个口罩反复消毒高达20次，且不影响口罩的防护功能。</span></p><section><br  /></section><section><strong><span>然而，现在的一次性口罩原本就不是为了重复使用而设计的，早期口罩为了重复使用、提高防菌效率的一些精心、重要的设计更是可能没有被纳入现代设计。</span></strong><span></span></section><section><br  /></section><section><span>无论如何，在可以预见的未来，口罩仍然将会是重要的个人防护设备。经过这次口罩在全球的广泛使用，人们会习惯于一次性口罩囤货，还是也会有可重复使用口罩的复兴？或许，口罩还会迎来新的改进。</span></section><section><br
