国家、地方医保局连续发文，耗材市场迎来变革？

data-bgopacity="30%"><span>一则是5月28日，国家医保局官网发布副局长陈金甫主持召开了高值医用耗材集中采购改革座谈会的消息，这有可能是高值耗材开展全国集采的信号。另一则是<span>6月1日，黑龙江医保局发布《黑龙江省药品和医用耗材集中采购及价格监督检查办法》（征求意见稿）</span>，<span>对药品和医用耗材集采中公立医疗机构和药品、医用耗材生产经营企业的行为做了明确的要求。</span></span></section><section data-bgless="spin" data-bgopacity="30%"><br  /></section><p><p><img src="image/20201014/077122c7b660115637fe9239ff6800be_2.png" /></p></p><p><br  /></p><p><span>5月初，国家医保局局长胡静林在天津进行的一次调研中便透露了一些信息：调研期间，胡静林出席了天津等九省份联盟采购高值医用耗材申报信息公开大会。他表示，<strong>本次天津、北京、河北、辽宁等省区市联盟采购人工晶体，是继去年安徽、江苏高值医用耗材集中带量采购破冰后又一重大进展。</strong></span></p><p><span><br  /></span></p><p><span>更重要的是，胡静林表示希望联盟地区以本次采购为起点，将更多质优价宜产品纳入采购范围，为全国普遍开展高值医用耗材集中带量采购探索经验。可见，<strong>国家医保局有意将高值耗材的集采推广至全国，但是是由国家组织还是地方自行安排，目前仍然未知</strong>。</span></p><p><span><br  /></span></p><p><span><strong><span>黑龙江医保局发布的文件则可以看做是对这一工作的回应</span></strong></span></p><p><br  /></p><p><span>《征求意见稿》明确，参加药品及耗材采购投标的投标人以低于成本的报价竞标，或者以欺诈、串通投标、滥用市场支配地位等方式竞标的，由县级以上人民政府医疗保障主管部门责令改正，没收违法所得；</span></p><p><br  /></p><p><span>中标的，中标无效，处中标项目金额千分之五以上千分之十以下的罚款，对法定代表人、主要负责人、直接负责的主管人员和其他责任人员处对单位罚款数额百分之五以上百分之十以下的罚款；情节严重的，取消其二年至五年内参加药品及耗材采购投标的资格并予以公告。</span></p><p><br  /></p><section><span>医用耗材生产经营企业具有11种行为之一的，将取消所有产品入围资格，并自取消之日起两年内，不接受其任何产品集采申请，不得以任何形式采购其产品，原签订的购销合同终止</span></section><section><br  /></section><ul><li><p><span>经执法执纪机关认定，在高值医用耗材购销活动中存在商业贿赂行为的；</span></p></li><li><p><span>提供虚假、无效文件的；</span></p></li><li><p><span>以其他方式弄虚作假，骗取入围的;</span></p></li><li><p><span>以低于成本的价格恶意投标，扰乱市场秩序的；</span></p></li><li><p><span>对入围品种擅自涨价或变相涨价的；</span></p></li><li><p><span>不配送或不按时配送入围品种，影响医疗机构临床诊治的；</span></p></li><li><p><span>经医疗机构验收确认，配送的高值医用耗材规格、包装等信息与入围品种规格、包装等信息不一致并不同意更换的；</span></p></li><li><p><span>以入围品种外高值医用耗材取代入围品种进行配送的；</span></p></li><li><p><span>违反现行医疗器械价格管理规定的；</span></p></li><li><p><span>恶意申投诉，扰乱集中采购正常秩序的；</span></p></li><li><p><span>集中采购管理机构规定的其他情形。</span></p></li></ul><p><span><br  /></span></p><p><span><strong><span>对于高值耗材的治理，国家也早有部署</span></strong><span><br  /></span></span></p><p><span><br  /></span></p><p><span>去年，国办印发了《治理高值医用耗材改革方案》，明确高值医用耗材将迎来带量采购。随后，在国家医保局的指导下，安徽、江苏率先对部分高值医用耗材进行带量采购。</span></p><p><span><br  /></span></p><p><span>但不少观点都认为，<strong>高值医用耗材目前没有建立统一标准和分类，品种规格繁多，市场分散，如何带量采购？</strong></span></p><p><span><br  /></span></p><p><span>对于这一棘手问题，在一场国务院政策吹风会上，国家医保局副局长李滔曾表示，<strong>国家医保局成立伊始就启动了全国统一的高值医用耗材编码编制制定工作，形成全国的通用语言，研究制定了医用耗材在内的15项信息业务的编码标准</strong>。去年6月27日已经在国家医保局的官网上线运行。<strong>其最大的特点是规范了医用耗材的分类目录，实现了“一品一码”。</strong></span></p><p><span><br  /></span></p><p><span>值得注意的是，李滔表示，下一步考虑，凡是产品信息不按照统一的分类和编码进入医保系统的，以后就不能在全国各省和国家平台上进行招采，也不能进行医保的支付。如果这一政策执行，将为统一编码提供强大的推动力。</span></p><p><span><br  /></span></p><p><span>目前，国家医保局推动的耗材集采正在逐步扩大试点范围。在去年底回复人大代表建议时国家医保局表示，将选取重点品种实施“带量采购”的试点工作，降低高值医用耗材的价格，惠及百姓。</span></p><p><span><br  /></span></p><p><span>去年7月，安徽以公立医疗机构采购联合体为主体，联合对骨科植入（脊柱）类、眼科（人工晶体）类产品带量采购。采购量为2018年度两类耗材采购量的70%、90%，同时规定医疗机构采购使用量不得低于2018年度的80%。骨科脊柱类材料总体平均降价53.4%，单个组件最大降幅95%，人工晶体类总体平均降价20.5%。预计两类产品年节约资金分别为3.7亿元和0.29亿元。</span></p><p><span><br  /></span></p><p><span>在江苏，55家三级公立医疗机构组成的联合采购联盟，拿出全年用量的70%采购，谈判的品种为雷帕霉素及其衍生物支架和双腔起搏器。支架按单个品种谈判，中选品种平均降幅51.01%，最高降幅66.07%；起搏器按生产企业谈判，中选企业涉及品种平均降幅15.86%，最高降幅38.13%。</span></p><p><span><br  /></span></p><p><span>国家医保局医药价格和招标采购司司长钟东波表示，两省此次试点带量采购的规模不算大，但效果很明显。</span></p><p><span><br  /></span></p><p><span>今年，天津、北京、河北、辽宁等省区市联盟采购耗材、内蒙古印发高值医用耗材改革实施方案，各省都在稳步推进相关工作，国家医保局也明确表示不会照搬4+7模式，还要结合高值医用耗材自身的一些特点来研究制定相应的集中带量采购方案。</span></p><p><span><br
