powered-by="gulangu"><section><section><section><br  /></section><section><span>左心耳形成的血栓易脱落，栓子随血流运往全身。脑栓塞是房颤患者致残率最高的并发症，一半以上的血栓会掉入脑血管，引起脑栓塞，导致偏瘫甚至死亡，目前大约每6个脑卒中患者中就有1个是房颤患者。有的血栓会造成肾栓塞，还有的血栓会造成肠梗死，或者是下肢坏死。以上疾病都会极大的造成患者痛苦，加重家庭的负担。</span></section><section><br  /></section><section><p><img powered-by="gulangu"><section><section><span>目前常用的预防左心耳（从左心房伸出的耳状小囊，属于左心房的一部分，它是左心房主要组成部位，<span>有研究指出，57%的瓣膜性房颤和90%的非瓣膜性房颤血栓均来自左心耳</span>）血栓形成的手段，多以口服华法林（抗凝药）为主，但使用华法林时需定期抽血监测INR，患者的依从性较差；也可使用新型口服抗凝药，但价格较贵。口服抗凝药均有出血的风险。对于不能耐受口服药，或者是出血风险高的患者，可以选择左心耳封堵术，是目前替代口服抗凝药的最好办法。</span></section></section></section></section></section></section><section><br  /></section><section><span><span>左心耳封堵是将封堵器固定在心耳处，从而阻止血流进入左心耳及左心耳内血栓脱落，</span>降低房颤患者由血栓栓塞引发长期残疾或死亡的风险。可消除患者对长期口服抗凝治疗的依赖性，为患者提供治疗新选择。</span></section><section><br  /></section><section><p><img data-style-id="22631"><section><section><p><strong>02</strong></p></section><section><p><span><strong>WATCHMANTM&nbsp;FLX</strong></span></p></section></section></section><section><br  /></section><section><span>WATCHMANTM左心耳封堵术由波士顿科学研发，于2006年在欧洲获批上市，2013年12月31日，正式获得中国国家食品药品监督管理总局上市批准，并于2014年3月在中国正式上市。这种手术通过微创，闭合了房颤病人血栓发生的根源部位——左心耳，由此可降低房颤病人中风的风险。</span></section><section><br  /></section><section><span>新一代产品WATCHMANTM FLX全圆形设计为医生提供了安全进入左心房附件并在其中活动的能力。与前代设备相比，它有更大的尺寸选择，可以更多的患者。同时WATCHMANTM FLX有一个新的框架设计，可以实现与组织的最佳接合，以获得长期稳定性和更快、更完整的密封。该产品于2019年3月，在欧洲获得了CE认证。</span></section><section><br  /></section><section><video type="video/mp4"></source></video></section><section><br  /></section><section><span>PINNACLE FLX研究是一项非随机的单臂临床试验，纳入美国29个中心的400例患者，研究结果显示新一代WATCHMAN FLX植入成功率高达99%；与NESTED、CAP2、PREVAIL以及CAP试验相比，WATCHMAN FLX发生安全性终点事件概率显著降低，仅为0.5%，进一步证实了WATCHMAN FLX的安全性。</span></section><section><br  /></section><section data-support="96编辑器" data-style-id="22631"><section><section><p><strong>03</strong></p></section><section><p><strong>中国LAAC的发展现状及未来前景</strong></p></section></section></section><section><br  /></section><section><span>关于LAAC在中国的发展，复旦大学附属中山医院葛均波院士于“WATCHMAN LAAC中美线上研讨会”上提到：对于预防房颤卒中，降低体循环栓塞的风险至关重要，临床中可选择抗栓治疗、消融、外科或介入手术。自2014年WATCHMAN在中国上市至今，已超过10,000例患者接受WATCHMAN LAAC。虽然自2019年底开始受COVID-19疫情影响，但至今年7月的数据显示也已有700例患者接受WATCHMAN LAAC。<br  /></span></section><section><br  /></section><section><span>近年来，大量的循证医学证据证实了LAAC的临床疗效和安全性。2018年中国专家共识将LAAC列为IIa类推荐；2019年ACC/AHA/HRS在更新的房颤管理指南中指出，对于具有高卒中风险、长期抗凝治疗禁忌证的NVAF患者，推荐使用 LAAC（IIb）。2019年中国连续发布了三项左心耳封堵专家共识，有效指导基层医院LAAC手术的工作开展。目前中国WATCHMAN相关临床数据显示：WATCHMAN手术成功率高达94.51%；患者平均CHA2DS2-VASc评分3.9分，平均年龄超过65岁，50%的患者为阵发性房颤患者。但临床中仍有48%的患者接受WATCHMAN LAAC进行二级预防，临床医师应更重视卒中二级预防以外的患者群体，使更广泛的患者获益；因此，未来LAAC的进一步发展仍需要多学科合作，并且需要个体化治疗房颤。</span></section><p><br  /></p><p><span>来源：哈特瑞姆心脏之声、波士顿科学、门诊新视野等</span></p><p><br  /></p></section></section></section></section></section><section><section
