/></p><p><span>3D打印也叫增材制造技术，是快速成型技术的一种。在医疗应用中，使用3D打印的方法成型生物材料，特别是细胞材料，用来制造人工的组织、器官，还有各种假肢，手术导板等一系列生物医疗领域的产品。随着我国医疗水平的提升，这项技术也越来越多的应用与我国医疗项目中。</span></p><p><br  /></p><p><span><strong><span>国内首例3D打印模型导航腹膜后巨大脂肪肉瘤切除术</span></strong></span></p><p><br  /></p><p><span>近日，广东省中医院结直肠外科刁德昌主任团队通过3D人体打印技术，成功完成国内首例3D打印模型导航腹膜后巨大脂肪肉瘤切除术，肿瘤体积达到40cm×30cm×15cm，重量达16斤。</span><br  /></p><section><span><br  /></span></section><p><p><img /></p><p><span>腹膜后巨大肿瘤被称为腹部的“定时炸弹”，其体积巨大，结构复杂，侵犯腹部多个脏器以及大血管，不仅给患者带来难以忍受的痛苦。由于手术难度较大，所以在手术前采用了3D人体打印技术重建模型技术，通过3D打印模型，能够清晰地看到，肿瘤的结构和与周围脏器的关系。</span></p><p><span><br  /></span></p><section><p><img src="image/20201014/a1c4107fb37eecfb3be760a96364b5e5_3.png" /></p></section><section><span>肿瘤模型3D打印过程</span></section><section><span><br  /></span></section><p><span>7月15日上午10时手术正式开始，术中采用“T”字形切口打开腹腔，按照四周向中心包绕、层层递进的原则，首先松解肿瘤表面与结肠的粘连，层层深入分离保护右侧输尿管；接着从肿瘤左侧松解粘连带，分离出左侧输尿管、腰神经，接着再从肿瘤头侧由浅入深，结扎肿瘤营养血管以及切除受到侵犯的腰大肌；最后在盆腔处钝性分离被肿瘤包绕的髂动脉，完整取出肿瘤。<br  /></span></p><p><span><br  /></span></p><section><span>由于术前利用3D人体打印技术，手术团队对肿瘤与周围组织的空间关系提前做足了功课，手术每一个步骤、需要避开的危险都了然于胸，整个手术在紧张专注的环境中流畅完成。耗时仅2个小时，术中出血仅不到100毫升。</span></section><section><span><br /></span></strong></span></p><section><br  /></section><section><span>虽然广东省中医院这台手术是国内首例3D打印模型导航腹膜后巨大脂肪肉瘤切除术，但在3D打印模型导航手术的其他领域，国内早有涉猎。在3D打印模型的手术引导下，手术损伤下降，手术时长缩短，广受好评，而这只是3D打印在医疗领域的一个应用。</span></section><section><br  /></section><p><span>现阶段，医疗3D打印在医疗领域的应用主要包括：血管打印、细胞打印、组织工程支架和植入物打印、假体打印和手术器械打印等生物3D打印的各个层面提出目前现状、存在的问题与未来的可能路径，从构造单元层面上提出了生物3D打印的五个阶段：</span></p><p><br  /></p><p><p><img /></p><p><span>3D生物打印技术使用异质材料、功能梯度材料及多尺度材料，打印各种空心、多孔、网格等结构与构造，打印精度可达≤0.1mm，在个性化医疗和模拟人体组织器官结构功能方面具有明显优势。</span></p><p><br  /></p><p><span>世界范围看，2015 Wholers报告显示，2014年增材制造下游应用领域医疗行业的占比为13.1%，仅次于消费电子、汽车与航空航天领域。2014年包括颅骨、髋骨、膝盖和脊柱等超过20项3D打印植入物获得FDA批准，已有10万多个髋关节植入物进行生产，并约有5万个应用到病人身上。根据Smar Tech Markets的研究预测，2015年，牙科与医学领域的3D打印市场规模可达到14.08亿美元，2020年有望达到45.44亿美元。</span></p><p><br  /></p><p><span>从20世纪90年代初，我国开始了3D打印研究，在3D生物打印基础研究领域基本与世界同步，但国内该领域科研团队或企业多集中在金属材料或无机非金属材料，对3D生物打印的基础性研究集中在个别高校。</span></p><section><br  /></section><section powered-by="gulangu"><section><section powered-by="gulangu"><p><strong><span>结语：</span></strong><span>当前3D生物打印已跨过第一、第二阶段，在医疗模型、诊疗器械、康复辅具、假肢、牙齿及人工关节等方面催生出了一个产业链雏形。我国政府也正加快解决3D生物打印技术应用面临的标准化问题，2020年3月5日，国家标准委等6部门联合印发了《增材制造标准领航行动计划（2020-2022年）》，将行动目标确定为到2022年，基本建立3D打印新型标准体系，相信未来该领域在政策加持下会得到快速发展。</span></p><p><br
