事关3.3亿人，医保报销将发生重大变化！普通门诊可报销，单位缴费不再划入个人账户

/></p><p><span>8月26日，国家医保局发布《关于建立健全职工基本医疗保险门诊共济保障机制的指导意见（征求意见稿）》。</span></p><p><br  /></p><blockquote data-type="2" data-url="" data-author-name="" data-content-utf8-length="144" data-source-title=""><section><section>征求意见稿提出，建立完善普通门诊医疗费用统筹保障机制，从高血压、糖尿病等群众负担较重的门诊慢性病入手，<strong>逐步将多发病、常见病的普通门诊医疗费纳入统筹基金支付范围。普通门诊统筹覆盖全体职工医保参保人员，支付比例从50%起步</strong>，随着基金承受能力增强逐步提高保障水平，待遇支付可适当向退休人员倾斜。</section></section></blockquote><p><br  /></p><p><span>征求意见稿还提出，规范个人账户使用范围。改革后，个人账户享受人群将从参保人本人扩大到其配偶、父母、子女。并且，还拟改进个人账户计入办法，原本单位缴费划入个人账户的部分将全部计入统筹基金。</span></p><p><br /></p></p><p><span><strong>拟扩大统筹基金支付范围</strong></span></p><p><br  /></p><p><span>我国职工医保制度于1998年建立，从建立之初就明确了医保基金实行统筹基金与个人账户相结合的方式，即统账结合。职工个人缴纳的基本医疗保险费，全部计入个人账户。用人单位缴纳的基本医疗保险费分为两部分，一部分用于建立统筹基金，一部分划入个人账户。</span></p><p><br  /></p><p><p><img /></p><p><span>两者的分工也不同。南开大学卫生经济与医疗保障研究中心主任朱铭来表示，统筹基金用于风险共济，包括住院看大病等。个人账户则主要用于个人支付，有对门诊费用进行偿付的功能。</span></p><p><br  /></p><p><span>随着经济社会的快速发展，医疗费用逐渐上升，现阶段其保障功能难以适应群众更充分的需求。</span></p><p><br  /></p><p><span>朱铭来指出，大多数二三线城市，没有较好的门诊统筹，门诊费用较高的群体负担较重。部分人在经济负担重、不愿意全额自费的情况下，就出现了一些明明是门诊疾病，却要办理住院，使用统筹基金的情况，这实际上是对医疗资源的浪费。</span></p><p><br  /></p><p><span>如何解决这个问题？朱铭来指出，一方面要降低患者经济负担，同时也需要和住院统筹形成一个有效衔接，增强门诊共济保障功能。</span></p><p><br  /></p><p><span>医疗战略咨询公司Latitude Health创始人赵衡对记者表示，中国的医保账户储蓄特性虽然符合文化特点，但其实在门诊支出上，并没有保险的保障功能，只是用户在支出自己历年的账户余额。一旦用户出现较高的医疗费用，比如慢性病或急症住院，需要长期服药和治疗，甚至需要一些自费药和项目，医保储蓄账户很难支持。门诊统筹的推进可以在很大程度上推动医保的共济能力。</span></p><p><br  /></p><p><p><img /></p><p><span>此次征求意见稿提出，改进个人账户计入办法。科学合理确定个人账户计入办法和计入水平，在职职工个人账户由个人缴纳的基本医疗保险费计入，计入标准原则上控制在本人参保缴费基数的2%以内，<strong>单位缴纳的基本医疗保险费全部计入统筹基金。</strong></span></p><p><br  /></p><p><p><img /></p><p><span>过去，医保个人缴费的2%和单位缴费的30%计入个人账户。朱铭来认为，这相当于将单位缴纳的原本划入筹资变成了门诊共济，这将增加门诊统筹的保障力度。</span></p><p><br  /></p><p><span>改革后，个人账户原有的钱会变少吗？医保待遇会下降吗？</span></p><p><br  /></p><p><span>国家医保局有关负责人回应，不仅不变，门诊保障待遇还能提升。改革后，参保人个人账户现有的钱不变，归个人使用，当期新划入个人账户的钱减少，用来加强门诊保障；统筹基金与个人账户的结构更加优化，保障能力更强；门诊医疗费用报销水平提高。</span></p><p><br  /></p><p><span>征求意见稿还提出，规范个人账户使用范围。<strong>个人账户主要用于支付参保职工在定点医疗机构或定点零售药店发生的政策范围内自付费用。可以用于支付职工本人及其配偶、父母、子女在医保定点医疗机构就医发生的由个人负担的医疗费用</strong>，以及在定点零售药店购买药品、医用耗材发生的由个人负担的费用。</span></p><p><br  /></p><p><span>同时，探索个人账户用于配偶、父母、子女参加城乡居民基本医疗保险等的个人缴费。个人账户不得用于公共卫生费用、体育健身或养生保健消费等不属于基本医疗保险保障范围的其他支出。健全和完善个人账户使用管理办法，做好收支信息统计。</span></p><p><br  /></p><p><p><img /></p><p><span>朱铭来对每日经济新闻（微信号：nbdnews）记者表示，这利于统筹基金与个人账户的结构优化，增加门诊统筹的保障力度。同时也将直接打通并且规范直系亲属范围内的共济，从而实现个人账户资金使用更加理性化、人性化。</span></p><p><br  /></p><p><span>朱铭来表示，此前也存在父母用个人账户在药店给孩子等亲属拿药等行为，与其不清不楚、不明不白使用个人账户资金，不如直接打通并且规范直系亲属范围内的共济，从而实现个人账户资金使用更加理性化、人性化。</span></p><p><br
