Skyra是新一代高端3T超导型磁共振成像系统，也是医院进行影像诊断的主力机型，临床诊疗有赖它的高效稳定运行。影像设备的精准成像，是开展精准医疗的基础。</span></section><section><br  /></section><section><span>近日，西门子医疗收到北京某三甲医院的求助：该院的Skyra磁共振成像系统扫描出来的腹部及手部图像，无法进行临床诊断，操作技师开始怀疑自己操作技能和设置的扫描参数有问题，希望原厂能来帮助解决。</span></section><section><br  /></section><section><span>目前这台设备的日常维护，是由国内的一家独立MR维修服务公司提供。医院为了节约成本，选择与该公司签订了5年金保合同，目前设备仍在合同期内，但图像质量已经让医院无法忍受。</span></section><section><span>&nbsp;</span></section><section><span>西门子医疗接到医院的需求后，安排有十年MR磁共振成像系统维修经验的唐日平工程师上门检修。唐工到达现场后，眼前的一幕让他震惊。仅仅对Skyra 磁共振系统部分功能和部分线圈进行检查，就发现诸多问题：</span></section><section><br  /></section><section><strong><span>体线圈调谐不过，一个通道发射频率超出系统范围，可能原因是双通道体线圈中的BC1通道已坏，需要更换发射体线圈。此问题将造成射频场不均匀，图像左右或前后明暗度不一致，同时对压脂造成影响；</span></strong></section><section><strong><span><br  /></span></strong></section><section><strong><span>梯度线圈内温度传感器GC8损坏，有更换梯度线圈风险（不能超过3个传感器损坏）；<br  /></span></strong></section><section><strong><span><br  /></span></strong></section><section><strong><span>机柜内2个水冷风扇损坏，需要更换；</span></strong></section><section><strong><span><br  /></span></strong></section><section><strong><span>系统伪影检测不过，影响图像质量；</span></strong></section><section><strong><span><br  /></span></strong></section><section><strong><span>核磁内循环水泵运行报过载，需要维修更换；</span></strong></section><section><strong><span><br  /></span></strong></section><section><strong><span>两个表面体线圈都已损坏，其中一个有2个通道损坏（B31/B32）,另一个有5个损坏（B21/B22/B31/B32/B35），此线圈经3方反复破坏性维修，始终无法修好，同时造成该线圈外观损坏；</span></strong></section><section><br  /></section><p><strong><span>大柔线圈2个通道损坏（FL2/FL4）；</span></strong></p><p><strong><span><br  /></span></strong></p><p><strong><span>头颈线圈5个通道损坏（H44/N11/N12/N21/N22）；</span></strong></p><p><strong><span><br  /></span></strong></p><p><strong><span>腕关节线圈2个通道（W36/W37损坏）；</span></strong></p><p><strong><span><br  /></span></strong></p><p><strong><span>膝关节线圈检测报错，同时引发设备报错，不能使用……</span></strong></p><section><br  /></section><section><span>当时的设备状态是：MR所有质量保证都没有做，ERDU接地电阻失超管等安全检查保修期后没有进行，不排除梯度射频及重建等其他问题，存在医疗事故等诸多安全隐患；重建计算机及主台计算机速度慢老化没有更新，系统随时可能停机；由于系统存在诸多问题，图像质量无法满足临床要求，极易引发误诊。</span></section><section><br  /></section><section><span>唐工面对设备惨状，感叹不已：<strong>莫将李鬼当李逵，十个线圈七个坏</strong><strong>，无良维保太坑人，宝马修成拖拉机！</strong></span></section><section><br  /></section><section><span>该设备由于第三方野蛮粗放维修导致的图像问题有：</span></section><p><br  /></p><p><p><img error代表有问题）</span></section><section><br  /></section><section><span>北京这家三甲医院的遭遇，在西门子医疗客户服务部遇到的维修案例中并不是个案，它折射出医疗设备维修市场的许多深层次问题。以MR产品为例，一些独立维修服务公司在承保前夸大宣传自己的维修及备件供应能力，承保后从成本角度考虑，对线圈，风扇等常用备件不做周期性更换，导致设备疲劳工作；对于射频发射体线圈及梯度线圈这样的高值备件更是无法提供，一旦高值备件损坏便逃之夭夭，留下的烂尾由医院买单。</span></section><section><br  /></section><section><span>针对市场乱象，西门子医疗呼吁在国家层面：<strong>建立医疗设备维修服务的一致性评价体系。医学影像设备无论由谁负责维修，都应当将修复后图像质量是否符合临床需要作为评判标准，并对维修行为进行溯源管理。</strong></span></section><section><br  /></section><section><span>同时我们建议医疗设备的使用单位，在选择设备承保公司时，要多维度考察承保公司的质量体系、人员能力、备件保障、专用工具等各个方面，谨慎选择。以MR磁共振成像系统为例，需要做的四个方面的保证：</span></section><section><br  /></section><section><span><strong><span>质量体系保证：</span></strong></span><span>MR磁共振成像系统为由磁体、射频、梯度、控制、计算机、病床等系统组成，每个系统又由多个子系统组成，每台设备的备件数量（非元器件数量）数以千计。诸多高新技术在MR上的运用，涉及了高电压、超低温、机械运动、强磁场、噪声、激光等诸多领域，而这些技术的使用都有严格的安全标准和质量控制标准，需要严格的质量控制体系，才能确保MR系统工作正常。</span></section><section><br  /></section><section><span><strong><span>人员能力保证：</span></strong></span><span>西门子医疗系统有限公司拥有100名MR专业工程师，全国中大型城市都有驻地工程师，均需参加西门子相应产品培训并获得服务资质证书方能上岗。其中：团队中52%的工程师是具备5年以上服务的高级工程师，团队中10%以上是专家级工程师。每个工程师每年将安排1-2门产品课程、由西门子（全球）培训中心进行培训并进行资格认证。此外，每个西门子医疗工程师背后，都有西门子全球技术中心提供技术支持，疑难杂症迅速解决，保证开机率。</span></section><section><br  /></section><section><span><strong><span>备件供应保证：</span></strong></span><span>西门子医疗在北京和上海设立备件保税仓库，备件储存超万种，货值超过
