data-id="99636"><section><section><section><p><span>每年9月至10月，是接种流感疫苗的最佳时机。</span></p><p><br  /></p><p><span>受新冠肺炎疫情影响，今年秋冬流感疫苗接种需求比往年同期有所增加。业内人士认为，接种流感疫苗或可减少因罹患流感而到医院就诊的人数，避免流感和新冠肺炎疫情叠加，减轻防控压力。各地应加大流感疫苗的生产调配，提前安排接种人员和场地，<strong>合理应对今年的流感季</strong>。</span></p><p><br  /></p><p><span>流感的主要症状是发热、咽痛、咳嗽、全身肌肉酸痛，极个别病例伴有消化道症状，发热和呼吸道症状与新冠肺炎没有特异性区别。<strong>单从这些症状来看，人们无法直接区分流感与新冠肺炎。</strong></span></p><p><br  /></p><p><span><strong>专家提醒，按照各地流感高发季节情况，广大市民于１０月前完成接种为宜</strong>。甘肃省疾控中心免疫规划科副科长刘舒瑜说，尽早接种既能避免扎堆，又能保证在流感季到来前，接种者有足够时间产生抗体。</span></p><p><br  /></p><p><br  /></p></section></section></section></section><section hm_fix="310:305"><p><span><strong>王辰院士等强调接种流感疫苗的必要性</strong></span></p></section><section><p><br  /></p></section></section></section></section><p><span><span>早在今年两会期间，国家援鄂医疗队成员、北京医院大内科主任孙铁英向公众介绍了自己在武汉抗疫一线的发现，<strong>50例重症COVID肺炎病人中，合并流感有18例</strong>。医生常常受困于“</span><span>很难区分清楚哪些是COVID、哪些是流感的病例</span><span>”。而随着秋冬季流感季的到来，这一现象将更为普遍。</span></span></p><p><br  /></p><p><span>在8月14日上午的第22届中国科协年会上，中国工程院副院长、中国医学科学院院长、北京协和医学院校长、中国工程院院士王辰在开幕式上以《常态与应急相结合——当前新冠防控策略》为题分享了他对新冠疫情发展和防控的措施建议。</span></p><p><br  /></p><section /></p></p><p><br  /></p><p><span>他指出，<strong>除了新冠疫情，还必须考虑到秋冬季节的时候会不会和季节性流感</strong>，会不会和一些新出现的流感等呼吸道传染病出现<strong>伴随出现、交互影响的情况，进而增加它的复杂性</strong>。</span></p><p><br  /></p><p><span>新冠一旦合并流感，鉴别诊断难度增加，隔离人群的社会负荷会明显加大，社会资源的耗费将显著增加，人群死亡率会上升。实际上，每年死于流感的人数并不少，但若和新冠交织在一起，对民众心理、社会稳定的冲击不可小觑。</span></p><p><br  /></p><p><span>他强调，在传染病的流行上，社会状况是至关重要的，自然环境的变化也是至关重要的，而且不同种类的传染病，特别是历史上的，已经不止一次出现了多种传染病交互存在的情况，这是人类最难防的。这一点，我们必须深存戒惧之心，未雨绸缪。</span></p><p><br  /></p><p><span>对此，他给出一个很重要的建议——最有效的办法是及时普遍接种流感疫苗。我希望全国充分考虑这个问题，一定是在今年9月底以前，不要晚于10月份，要进行充分的流感疫苗大人群接种。对流感疫苗，特别是能涵盖A、B型流感的四价疫苗的产能要迅速安排提升。</span></p></section></section></section></section><section hm_fix="310:305"><p><span><strong>流感疫苗应如何选择及接种</strong></span></p><p><br  /></p></section></section></section></section><p><span>流感疫苗该怎么打？谁能打呢？</span></p><p><br  /></p><p><span>我国现已批准上市的流感疫苗有三价灭活流感疫苗(IIV3)和四价灭活流感疫苗（IIV4）， IIV3 包括裂解疫苗和亚单位疫苗，IIV4 为裂解疫苗。</span></p><p><br  /></p><p><span>其中IIV3有裂解疫苗和亚单位疫苗，可用于≥6月龄人群接种，包括0.25ml和0.5ml 2种剂型，0.25ml剂型含每种组份血凝素7.5μg，适用于6～35月龄婴幼儿；0.5ml剂型含每种组份血凝素15μg，适用于≥36月龄的人群；IIV4为裂解疫苗，适用于≥36月龄的人群，为0.5ml剂型，含每种组份血凝素15μg。</span></p><p><br  /></p><p><span>根据国家药监局网站和疫苗批签发信息，2019-2020 季节有 7 家厂家供应流感疫苗。此外，一种鼻喷三价流感减毒活疫苗在上市审批过程中，适用人群为3-17岁，每剂0.2ml。</span></p><p><br  /></p><p><span><strong>对可接种不同类型、不同厂家疫苗产品的人群，可自愿接种任1种流感疫苗，无优先推荐。</strong></span></p><section><section hm_fix="310:305"><p><span><strong>流感疫苗的优先接种人群</strong></span></p></section><section><p><br  /></p></section></section></section></section><p><span>流感疫苗安全、有效。原则上，接种单位应为≥6 月龄所有愿意接种疫苗且无禁忌证的人提供免疫服务。国内外大量流感疾病负担的科学证据表明，不同人群患流感后的临床严重程度和结局不同，借鉴 WHO 立场和其他国家多年的应用经验，结合我国国情，推荐以下人群为优先接种对象。</span></p><p><br  /></p><p><span><strong>6~23 月龄的婴幼儿</strong></span></p><p><br  /></p><p><span>患流感后出现重症的风险高，流感住院负担重，应优先接种流感疫苗。疫苗在该年龄组的效果高度依赖于疫苗株与循环毒株的匹配程度。</span></p><section data-id="98480" data-tools="gulangu"><p><br  /></p><p><span><strong>2~5 岁儿童</strong></span></p></section><p><br  /></p><p><span>流感疾病负担也较高，但低于 2 岁以下儿童。该年龄组儿童接种流感疫苗后，其免疫应答反应通常优于 2 岁以下儿童。&nbsp;<br  /></span></p><section data-id="98480" data-tools="gulangu"><p><br  /></p><p><span><strong>60 岁及以上老年人</strong></span></p><p><br  /></p></section><p><span>患流感后死亡风险最高，是流感疫苗接种的重要目标人群。虽然较多证据表明，现有流感疫苗在老年人中的效果不如年轻成年人，但疫苗接种仍是目前保护老年人免于罹患流感的最有效手段。&nbsp;<br  /></span></p><section data-id="98480" data-tools="gulangu"><p><br  /></p><p><span><strong>特定慢性病患者</strong></span></p></section><p><br  /></p><p><span>心血管疾病（单纯高血压除外）、慢性呼吸系统疾病、肝肾功能不全、血液病、神经系统疾病、神经肌肉功能障碍、代谢性疾病（包括糖尿病）等慢性病患者、患有免疫抑制疾病或免疫功能低下者，患流感后出现重症的风险很高，应优先接种流感疫苗。<br  /></span></p></section><section data-tools="gulangu"><p><br  /></p><p><span><strong>医务人员</strong></span></p></section><p><br  /></p><p><span>流感疫苗接种的重要优先人群，不仅可保护医务人员自身，维持流感流行季节医疗服务的正常运转，同时可有效减少医务人员将病毒传给流感高危人群的机会。&nbsp;<br  /></span></p><section data-id="98480" data-tools="gulangu"><p><br  /></p><p><span><strong>6 月龄以下婴儿的家庭成员和看护人员</strong></span></p></section><p><br  /></p><p><span>由于现有流感疫苗不可以直接给 6 月龄以下婴儿接种，该人群可通过母亲孕期接种和对婴儿的家庭成员和看护人员接种流感疫苗，以预防流感。&nbsp;<br  /></span></p><section data-id="98480" data-tools="gulangu"><p><br  /></p><p><span><strong>孕妇或准备在流感季节怀孕的女性</strong></span></p></section><p><br  /></p><p><span>国内外大量研究证实孕妇在孕期任何阶段接种流感疫苗的安全性证据充分，同时接种疫苗对预防孕妇罹患流感及通过胎传抗体保护 6 月龄以内婴儿的效果明确。另外，WHO 流感疫苗立场文件（2012 年版）将孕妇列为第一优先接种人群。但由于国内缺乏孕妇接种流感疫苗的安全性评价数据，我国上市的部分流感疫苗产品说明书仍将孕妇列为禁忌证。划重点！为降低我国孕妇罹患流感及严重并发症风险，经审慎评估，本指南建议孕妇或准备在流感季节怀孕的女性接种流感疫苗，孕妇可在妊娠任何阶段接种。</span></p><section 版）未将对鸡蛋过敏者作为禁忌。药典规定流感疫苗中卵清蛋白含量应不高于500ng/ml，当前疫苗中的卵蛋白含量已大大低于国家标准。国外学者对于鸡蛋过敏者接种 IIV 或 LAIV 的研究表明不会发生严重过敏反应。美国 ACIP 自 2016 年以来开始建议对鸡蛋过敏者亦可接种流感疫苗。&nbsp;</span></p><p><br  /></p><p><span>2.患伴或不伴发热症状的轻中度急性疾病者，建议症状消退后再接种。</span></p><p><br  /></p><p><span>3.上次接种流感疫苗后 6 周内出现吉兰-巴雷综合征，不是禁忌症，但应特别注意。<br  /><br  /></span></p><section><p><strong><span>其他注意事项</span></strong></p></section><p><br  /></p><section><p><span>1.如正在或近期曾使用过任何其它疫苗或药物，包括非处方药，请接种前告知接种医生。&nbsp;</span></p></section><p><br  /></p><section><p><span>2.灭活流感疫苗与其它灭活疫苗及减毒活疫苗可同时在不同部位接种；未发现影响流感疫苗和联合接种疫苗的免疫原性和安全性的证据。老年人可同时接种流感疫苗和肺炎球菌疫苗。&nbsp;</span></p></section><p><br  /></p><section><p><span>3.免疫抑制剂（如皮质类激素、细胞毒性药物或放射治疗）的使用可能影响接种后的免疫效果。为避免可能的药物间相互作用，任何正在进行的治疗均应咨询医生。&nbsp;</span></p></section><p><br  /></p><p><span>4.服用流感抗病毒药物预防和治疗期间可以接种流感疫苗。</span></p><p><br  /></p><p><span>疫苗如果需要打，总归是越早打越好，打得早，保护得早。</span></p><p><br  /></p><p><span>在新冠病毒尚未被人类制服时，还是先听专家的话去做吧！</span></p><p><br  /></p><section><br  /></section></section></section></section></section></section><section><section><section
