data-style-id="22631"><section><section><p><strong>01</strong></p></section><section><h2><span><strong><span>高值耗材带量集采常态化，降幅明显</span></strong></span></h2></section></section></section><section><br  /></section><section><span>目前来看，医用耗材带量采购已经是大势所趋。国家也不断释放强烈的政策导向，推动集中带量采购常态化。随着各省或省级联盟集采持续推进，从降幅来看取得不错的成果，以人工晶体、冠脉球囊、冠脉支架为例。</span></section><section><br  /></section><section><span><strong><span>1、人工晶体</span></strong></span></section><section><br  /></section><section><span>截至目前，人工晶体已成为全国带量采购覆盖率最高的品种。截至2020年9月，涉及人工晶体的带量采购项目共6个，包括两个省级联盟项目、三个省级项目、一个市级项目。</span></section><section><span>&nbsp;</span></section><section><span>项目覆盖安徽、江苏、北京、天津、河北、辽宁、吉林、黑龙江、山西、内蒙古、山东、宁夏、甘肃、青海、新疆、新疆建设兵团、湖南、广西、贵州、海南、陕西、云南曲靖、上海等22个省份，在全国31个省（市、区）中占比达到70.97%。</span></section><section><br  /></section><section><strong><span>从已公布的谈判结果看，安徽平均谈判降幅为20.5%；江苏平均降幅约26.89%，最高降幅38%；京津冀“3+6”联盟谈判后，平均降幅约53.72%，最高降幅84.21%。</span></strong></section><section><strong><span>&nbsp;</span></strong><span></span></section><section><span><strong><span>2、冠脉球囊</span></strong></span></section><section><span><br  /></span></section><section><span>截至2020年9月，涉及冠脉球囊的带量采购项目共5个，项目覆盖贵州、重庆、海南、湖北、云南、江苏、浙江、湖南等省份。</span></section><section><br  /></section><section><strong><span>从已公布的谈判结果看，云南平均谈判降幅在70%以上；江苏平均降幅为74.37%，最大降幅为81.05%；湖北省最高降幅96%；黔渝琼3省联盟预扩组平均降幅达85.49%，最高降幅为89.84%，后扩组平均降幅达85.15%，最高降幅为90.35%，总体降幅85.32%。</span></strong></section><section><br  /></section><section><span><strong><span>3、冠脉支架</span></strong></span></section><section><br  /></section><section><span>截至2020年9月，涉及冠脉支架的带量采购项目共4个，涉及江苏、山西等省份。</span></section><section><span>&nbsp;</span></section><section><span>从已公布的谈判结果看，江苏省平均降幅51.01%，最高降幅66.07%；山西省平均降幅52.98%，最高降幅69.12%。</span></section><section><span><br  /></span></section><section data-style-id="22631"><section><section><p><strong>02</strong></p></section><section><p><strong><span>耗材国采支架先行，最快或于9月启动</span></strong></p></section></section></section><section><br  /></section><section><span>继药品国家集采之后，高值耗材国家集采已经箭在弦上。<strong>首批国家级高值耗材集采将从冠脉支架入手</strong>。据业内人士分析，最快或于9月份启动。</span><br  /></section><section><br  /></section><section><span>今年7月，国家医疗保障局医药价格和招标指导中心发布了关于委托开展《国家组织冠脉支架集中带量采购方案(征求意见稿)》征求意见函。此次国采将从冠脉支架入手，探索开展国家组织高值医用耗材集中带量采购。</span></section><section><span>&nbsp;</span></section><section><strong><span>本次涉及的材质为钴铬合金和铂铬合金，载药种类为雷帕霉素或衍生物，药物载体涂层性质为非聚四氟乙烯的冠状支架</span></strong><span>。国家组织高值医用耗材联合采购办公室负责拟定具体技术指标。</span></section><section><br  /></section><section><strong><span>采购周期为2年</span></strong><span>，协议期满后，由各省确定供应企业、约定采购量和采购期限。供求关系、技术水平、市场格局发生重大变化的，可通过竞价、议价、谈判、询价等方式，产生中选企业、中选价和约定采购量。</span></section><section><br  /></section><section><span>从采购范围和数量来看，本次带量采购是目前有过的最大的采购联盟。方案提到，<strong>2019年冠脉支架使用量超过1000个的公立医疗机构、军队医疗机构均应参加</strong>，其他公立医疗机构、军队医疗机构、医保定点社会办医疗机构按照所在省份安排自愿参加。已开展该类产品集中带量采购且尚在合同期内的省份，可不参加。</span></section><section><br  /></section><section><span>在约定采购量比例上，参与联盟采购的医疗机构根据2019年采购量申报总采购基数和各产品采购基数（不含不锈钢支架）。按<strong>总采购基数的80%约定采购量</strong>，且保证每个中选产品采购量不低于申报采购基数的80%。</span></section><section><br  /></section><section><span>目前，<strong>冠脉支架领域已基本完成国产替代，其中主要外资企业包括美敦力、波士顿科学、强生等，在国内市场份额不足30%，主要内资企业包括微创、乐普、吉威、赛诺等，国内厂家合计市场份额已超过70%</strong>。其中微创医疗的市场份额最高，处于冠脉支架的龙头位置。值得注意的是，赛诺医疗的产品全是不锈钢材质，不符合采购条件，其他三家公司都符合条件的主力产品，均有参与集采的资格。</span></section><section><br  /></section><p><p><img hm_fix="238:230"><p><br  /></p></section></section></section></section><section><span>高值耗材集采的持续推进，将至少从两个维度加速国产替代。<strong>一方面，集采降价将导致外资利润大幅压缩</strong>，使其改变竞争策略。以往巨额利润导致外资并未在国内市场精细化管理。目前已有部分进口企业通过与国内企业合作来继续谋求生存空间。<br  /></span></section><section><br  /></section><section><span>另一方面，<strong>集采实施后标外产品的占比将被大比例压缩</strong>，而外资高端产品又大概率是标外产品。当标外产品的量遭到压缩后，最终出货量维系不了物流和仓储成本，将使外资被迫出局。</span></section><section><br  /></section><section><span>以骨科植入类耗材为例，骨科医疗器械按照治疗疾病种类不同可分为创伤类、脊柱类、关节类以及其他。据了解，创伤领域已基本完成进口替代，其次是脊柱市场，相较于创伤类和脊柱类产品，人工关节类器械是永久性植入产品，技术门槛高，制造工艺难且手术难度大，进口替代速度相对较慢，国产化程度最低，达26.73%。业内人士指出，带量采购或将加速国产企业在脊柱和创伤领域市占率的提升，行业将向具备规模优势、技术先进、创新迭代的头部企业集中。</span></section><p><br  /></p><section data-style-id="22631"><section><section><p><strong>04</strong></p></section><section><h2><strong><span>价格进一步回落，或再无经销商赚差价</span></strong></h2></section></section></section><section><br  /></section><p><span>高值医疗耗材因其技术含量、临床价值、学术推广和市场培育等要素，导致市场价格高昂。</span></p><p><br  /></p><section><span>集采如火如荼的开展，高值耗材的价格水分不断被挤压。据业内人士分析，预计耗材带量采购后，商业模式将发生较大变化。</span></section><section><br  /></section><section><span>由于高值耗材单价较高，出厂价和终端价之间差价较大，渠道中有较多“水分”，但在两票制和一票制的配合之下，耗材的水分将被再次压缩。也就是说集采的降价空间大部分由经销商承担。</span></section><section><br  /></section><section><span>另外，医疗器械产品的推广需要更多的多级经销体系，导致器械经销商小而分散。随着一票制的探索推行，或使生产企业参与到医院的供应链管理，使得经销商逐渐消失。</span></section><section><br  /></section><p><span>文章来源：Eshare医械汇</span></p><p><br  /></p><section><section
