分子诊断走出传统PCR实验室，才叫分子POCT！

data-autoskip="1"><p><span>随着新冠疫情在全球的快速蔓延，检测快速，操作简单的POCT（point-of-care testing，即时检测）备受大家关注。除了传统的胶体金技术外，分子POCT也逐渐为人所知。</span></p><p><br  /></p></section></section></section></section><section><span>例如国外雅培IDNOW（前Alere i平台，2017年被雅培收购），新冠病毒阳性结果最快可在5分钟内获得，阴性结果可在13分钟内确认，备受舆论关注。其它传统分子POC列强如丹纳赫旗下塞沛的GeneXpert，梅里埃的FilmArray，Biocartis的Idylla亦相继推出或宣布研发针对COVID-19的快速分子检测。国内企业如优思达、卡尤迪、奥然生物、万孚倍特等也借此风口陆续推出相关产品。</span></section><p><br  /></p><section><span>今年5月7日，中共中央政治局常委、国务院总理、中央应对新冠肺炎疫情工作领导小组组长李克强主持召开领导小组会议，会议要求，要进一步集中力量重点攻关，<strong>加快提高核酸检测能力，尤其是推进检测时间短、且无需实验室的核酸快速检测设备生产扩能，加大政策、审评审批等支持等</strong>。那么POC分子诊断目前还需要在传统普通PCR实验室开展吗？</span></section><section><br  /></section><section><span>近日，为指导医疗卫生机构检验实验室建设，推进《公共卫生防控救治能力建设方案》实施，国家卫生健康委、国家发展改革委制定了《医疗卫生机构检验实验室建筑技术导则（试行）》。该《导则》第七条规定：“核酸检测实验室可分为试剂准备区、样本制备区、核酸扩增区和产物分析区。结合实际，采用集中布置或分散布置形式，并配套设置洗消设施。<strong>当采用实时荧光定量PCR仪时，核酸扩增区和产物分析区可合并为一区。当采用一体化自动化核酸分析设备时，样本制备区、核酸扩增区和产物分析区可合并为一区。”</strong></span></section><p><br /></section><section><span>那是不是POC分子诊断就不需要在PCR实验室开展了呢？其实该规定在2010年原卫生部颁布的《医疗机构临床基因扩增检验实验室管理办法》的附件《医疗机构临床基因扩增检验实验室工作导则》就有相关体现：“原则上临床基因扩增检验实验室应当设置以下区域：试剂储存和准备区、标本制备区、扩增区、扩增产物分析区。这4个区域在物理空间上必须是完全相互独立的，各区域无论是在空间上还是在使用中，应当始终处于完全的分隔状态，不能有空气的直接相通。根据使用仪器的功能，区域可适当合并。例如使用实时荧光PCR仪，扩增区、扩增产物分析区可合并；<strong>采用样本处理、核酸提取及扩增检测为一体的自动化分析仪，则标本制备区、扩增区、扩增产物分析区可合并。</strong>”不过《医疗卫生机构检验实验室建筑技术导则（试行）》首次使用了“一体化自动化核酸分析设备”的概念。</span></section><section><br  /></section><section><span>之前较早公开的《病原微生物核酸扩增检验实验室能力通用要求行业标准（征求意见稿）》，则将按实验室活动和仪器类型的不同，实验室可分为普通PCR实验室、全自动定量PCR实验室、<strong>一体自动化分析PCR实验室</strong>、其他涉及核酸扩增的实验室等。其中同样明确规定<strong>一体化自动化分析PCR实验室通常设置二个工作区，即试剂准备区和自动检测区</strong>。与上述法规要求一致。</span></section><p><br  /></p><section><span>综合以上文件可以看出，涉及PCR的POC分子诊断仍需要在经认证的PCR实验室开展，但是POC分子诊断不需要传统普通PCR实验室的三间房或四间房，只需要设置试剂准备区和自动检测区两个分区即可。相比传统普遍PCR实验室的四个或三个分区，降低了空间要求，同时减少了配套设备要求。让更多中小实验室能够开展分子检测，提高基层公共卫生防控救治能力。</span></section><p><br  /></p><section><span>但是对于采用封闭式卡盒设计的产品，人工只需要简单加样，所有样本处理、核酸提取、PCR扩增和数据分析全由仪器自动完成。这时候试剂准备区仅有储存检测盒的功能，样本加样和仪器操作在自动检测区进行。这时候单独设置试剂准备区有些浪费空间，建议可以考虑与其它试剂储存功能区合并。另外，实验室如果本身是传统标准PCR实验室，则可以将检测盒储存在试剂准备区，在样本制备区加样，在扩增区上机。但以上都只是折中，仍与李克强总理提到的“<strong>检测时间短、无需实验室的核酸快速检测设备</strong>”仍有较大差距。真正的POC分子诊断应该是无论空间，随时随地能够开展的分子检测。</span></section><section><br
