powered-by="gulangu"><section><section><p><span>公立医院回款账期长是长期困扰民营医药商的一个问题。近日，据江苏《政风热线》栏目报道，江苏北部某公立二甲中医院拖欠一公司医疗器械货款6万余元。</span></p></section></section></section></section><section><section><br  /></section></section></section></section><p><span><strong><span>一批公立医院被曝拖欠货款</span></strong></span></p><p><br  /></p><p><span>公立医院回款账期长是长期困扰民营医药商的一个问题。近日，据江苏《政风热线》栏目报道，江苏北部某公立二甲中医院拖欠一公司医疗器械货款6万余元。</span></p><p><br  /></p><p><span>此前，双方的合同上没有约定付款时间，该市卫健委介入后，医院当即结清了这笔货款。</span></p><p><br  /></p><p><span>医院拖欠货款早已诸见报端。前不久莆田市第一医院（三甲医院）因长期拖欠供应商货款、耗材回款一拖再拖等问题被通报，该院财务科科长被通报批评，并调离岗位。</span></p><p><br  /></p><p><span>据《三湘都市报》报道，2016年仅湖南湘雅医院、湖南省人民医院、湖南省儿童医院等20家公立医院就拖欠了长沙40余家药商超过100亿元的药品货款。为了维持药品的正常供应，甚至有部分药商只好高利率向民间机构借贷，仅此部分损失每月就高达20万元。</span></p><p><br  /></p><p><span>2018年，湖北省医药行业协会在调研了8家有代表性医药商业企业发现，公立医疗机构药品回款账期最长达960天，欠款金额最高达8600万元。</span></p><p><br  /></p><p><span>而根据河南省审计厅2018年公布的部分公立医院财务收支情况审计结果显示：</span></p><p><br  /></p><p><span>郑州大学第三附属医院拖欠供应商半年以上药品、器材、设备款等超过1.25亿元；河南省胸科医院拖欠药品、试剂款近1.6亿元。</span></p><p><br  /></p><p><strong><span>国务院发文！30天内必须回款</span></strong></p><p><br  /></p><p><span>公立医院拖欠医药企业货款问题由来已久，有媒体甚至认为这是一种“行业规矩”。《看医界》专栏作者、山西省卫健局副局长徐毓才认为，医院拖欠货款的原因主要有两方面：一方面是给自己发展注入了资金，另一方面还是医院资金紧张，在长期负债经营的情况下，只能采取拖欠货款的办法维持自身的发展。</span></p><p><br  /></p><p><span>另外，面对药械、建筑企业，医院处在强势地位，供应商一般都不敢得罪，即使自己很困难也不敢怒。长此以往，医院拖欠货款逐渐变成“潜规则”。</span></p><p><br  /></p><p><span>面对医院拖欠货款的沉疴痼疾，国家早有政策规定。</span></p><p><br  /></p><p><span>2010年，卫生部、国务院纠风办、国家发改委、监察部等七部委联合印发的《医疗机构药品集中采购工作规范》规定：“医疗机构应当严格按照合同约定的时间回款，回款时间从货到之日起最长不超过60天。无正当理由未按合同规定时间回款的，应当支付一定比例的违约金。”</span></p><p><br  /></p><p><span>2015年2月，国务院出台《关于完善公立医院药品集中采购工作的指导意见》要求：“医院将药品收支纳入预算管理，严格按照合同约定的时间支付货款，从交货验收合格到付款不得超过30天”。</span></p><p><br  /></p><p><span>2017年2月，国务院办公厅发布的《关于进一步改革完善药品生产流通使用政策的若干意见》进一步提出：医疗机构要及时结算药款，对违反合同约定，无正当理由不按期回款或变相延长货款支付周期的医疗机构，卫生计生部门要及时纠正并予以通报批评、限期整改、责令支付违约金、降低等级等处理。</span></p><p><br  /></p><p><span>此外，今年7月国务院发布的《保障中小企业款项支付条例》也明确规定，机关、事业单位从中小企业采购货物、工程、服务，应当自货物、工程、服务交付之日起30日内支付款项，合同另有约定的，付款期限最长不得超过60日。对拒绝或者迟延支付中小企业款项的机关、事业单位，应当在公务消费、办公用房、经费安排等方面采取必要的限制措施，情况严重的，还将其失信信息纳入全国信用信息共享平台，依法实施失信惩戒。</span></p><p><span><br  /></span></p><p><p><img /></p></p><p><br  /></p><p><span><strong><span>医院拖欠货款如何从源头解决？</span></strong></span></p><p><br  /></p><p><span>徐毓才认为，一部分公立医院拖欠货款的原因，与自身运营困难有关。针对这个问题，2015年国办7号文已经给出了3条具体解决路径：</span></p><p><br  /></p><p><span>一是由财政支持设立药品采购周转金；二是医保部门将基金按药品费用支出比例提前预付给医院；三是鼓励通过互惠互利、集中开设银行账户，由银行提供相应药品周转金服务，加快回款时间。</span></p><p><br  /></p><p><span>从去年开始，在国务院意见的指导下，浙江、甘肃、福建等地已经开展了各种有益探索。</span></p><p><br  /></p><p><span>例如，浙江省实行医保基金预付医院两个月的医药费用，通过采购平台引入银行30～56天免息信用贷款。设立统一的省级药械联合采购监管账户，对医院的采购账户进行监管，医院在药品验收入库后，30天内从采购账户将货款打到监管账户，监管账户于次日再付款给供应企业。</span></p><p><br  /></p><p><span>甘肃省则计划统一引入一家银行作为第三方提供流动性融资，由银行根据省药品采购平台提供的采购数据，为药品供应商提供30天药款支付。</span></p><p><br  /></p><p><span>而福建省的做法是，全省定点公立医疗机构的药品货款由医保部门按月、统一与配送企业进行结算支付。</span></p><p><br  /></p><p><span>2020年3月，《中共中央国务院关于深化医疗保障制度改革的意见》出台，提出“探索医疗服务与药品分开支付”，言下之意就是药品、耗材费用由医保经办机构直接向供应商支付，而医疗服务费用向医疗机构支付。同时《意见》提出，有条件的地区可按协议约定向医疗机构预付部分医保资金，缓解其资金运行压力。</span></p><section><br  /></section></section></section></section></section></section></section></section></section></section></section><section><section
