height: auto;" /><br />高压注射器在医学影像科应用非常广泛，是DSA、CT、MRI等大型机器必不可少的设备之一。特别是行心脏、主动脉等DSA时需要的压力大，必须使用高压注射器。虽然该设备型号众多，但每个操作室仅配置一台高压注射器，使用过程中难免出现故障，面对患者数量多时，如何快速检修设备，提高使用效率就显得尤为重要。结合笔者多年的经验，现针对国内类似报道少、典型且有隐蔽性的故障之检修情况介绍如下。</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">1 故障现象</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">本单位选用的是Mark V ProVisTM型高压注射器，大型数字减影设备成像技术（APR）选择外周减影模式下，Mark V可以正常注射，APR选择心室或大血管团注的情况下不能正常注射，仅推进2～5 ml后，Mark V停止运行，设备无错误信息。</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">2 检查分析</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">根据上述故障信息，首先想到的是APR选择上有问题，在心脏模式下重新进行高压注射，查看数字减影设备Integris CV操作面板，亦无报错信息，初步判断此次高压注射故障与数字减影设备无关。</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">其次分析是否与联动有关，仔细检查联动线路，无破损、断路现象，尝试脱离联动注射，同样采用APR外周、心脏模式下不同的注射条件，外周模式下可以正常注射，心脏模式下仍仅完成少量注射。同时由于近距离操作注射，发现高压注射面板在注射不能正常完成时，Mark V操作控制面板上的对话窗短暂显示“UNIT DISARMED 05”。查操作手册，05代表“压力保护套”，于是检查压力厚壁套筒，发现有细微裂痕，更换压力套，重新注射，故障仍存在。</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">该设备已使用6年余，均采用专人专管，个人负责，严格操作规程等制度，以前并未出现过任何故障，而此次故障比较怪异，查阅大量资料和电话咨询厂家后，亦无类似情况。</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">重新理清思路，进行多次试验，对不同条件下的高压注射数据进行采集，见表1。 表1 不同条件下的高压注射数据对比发现，小压力下能够完成注射任务，大压力下仅注射2～5 ml后自动停止，而大C型臂的APR选择外周时需要的压力较小，心脏模式下压力较大，从而通过了外周时注射成功、心脏模式下失败的解释。也误导了最初的判断。掌握以上信息，寻求和压力有关的方面，在不影响造影效果的前提下，尝试降低注射速度、药物浓度，待药物加热到体温，去掉连接管，用粗些的导管进行试验，依旧未能解决问题。</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">既然操作和软件没有问题，只有考虑机械故障，但结合上述分析，似乎假设又不成立。经过反复的注射试验，最终发现大压力下注射时，高压注射器机头转动架固定不稳，在注射瞬间向前微动，细节引起了我们的注意，转动架应该与机头结合紧密，不应有缝隙。关闭高压注射器电源，小心的拆开注射机头外壳，去掉机头上的操作面板，显露出转动架轴心，发现转动轴心上的凹型卡轮碎裂，这应该是造成此类故障的直接原因。</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">由于空间小，虽然凹型卡轮碎裂，并没有完全与轴心脱离，当选择小压力注射时，推动力小，转动架动作微小，对高压针头影响不大，故而不影响注射；当使用较大压力注射时，推动力强，挤压凹型卡轮，使之变形、间隙增大，导致转动架不稳，高压注射器CPU误认为转动架没有到位，压力保护开启，工作停止，但由于启动注射开关反应时间差，瞬间的惯性，可使活塞推进少许，注射2～5 ml自动停止，造成了上述故障。</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">3 解决方法</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">将转动架拆除，用卡钳卸下转动架轴心的卡簧，记住安装位置，小心地从注射机头里抽出转动架轴心，取出转动架轴心上碎裂的凹型卡轮，已碎为三段。该卡轮采用硬塑料材质，因长期使用老化、变脆，装卸高压针筒时少用力震动、大压力注射均容易将其压裂。电话寻求厂家更换配件，却因生产地址在美国，且没有如此细小的配件，未果。尝试用AB胶将其小心黏合，24 h后原位放好，安装外壳、转动架等，通电试机，经不同压力下反复注射，高压注射器完全恢复正常工作。</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">4 讨论</p><p style="margin-top: 15px; margin-right: 18px; margin-left: 18px; line-height: 24px; color: rgb(51, 51, 51); font-family: 宋体, 宋体, Arial; font-size: 14px; white-space: normal; text-indent: 2em;">Mark V ProVisTM型高压注射器在大型C臂机、CT机等应用广泛，该高压注射器中的凹型卡轮采用硬塑料材质，其硬度和耐磨度差，长时间使用很容易老化，稍加外力就会碎裂。后得知厂家工程样机的凹型卡轮也系碎裂黏合而成。造成此故障具有一定的隐蔽性、欺骗性，后得知厂家工程样机的凹型卡轮也系碎裂黏合而成。在维修过程中应该仔细观察细节，积累经验，少走弯路。在使用过程中，也应该注意动作轻柔，切忌在更换高压针筒时硬推硬拽。</p><p style="margin-top: 15px;
