（案例分析）医院医疗设备维修与第三方服务机构合作得失分析

2em;">医疗设备不仅是开展医疗、教学、科研工作的必备条件，也是医院持续发展的重要保证。它大大提高了临床诊治能力和水平，给医院带来可观的经济效益和社会效益。目前国内三甲医院的医疗设备固定资产平均在3～8亿元，每年的维保费用以千万元计，如何在确保医疗质量与安全的前提下，提高医疗设备使用效率，降低维修成本，是医院管理者面临的一大挑战。医疗设备维修主要有三种模式：原厂维修、医院医学工程人员维修、第三方服务机构维修。<p 1.8em;text-indent: 2em;">在欧美发达国家，政府严格管控垄断行为，厂家受政策制约，无法形成垄断地位，第三方服务机构得到很好的发展，也为医院提供了更多选择。在国内，2015年CFDA18号令确立了第三方服务的合法性后，第三方服务机构发展迅速，其服务对象也逐渐由中小医院向三级甲等医院渗透，医疗机构与第三方服务机构合作呈扩大趋势。<p style="caret-color: 2em;">但第三方服务也存在发展瓶颈和乱状，一方面，一些大型进口设备厂商为争取售后利润，实行技术垄断，封锁技术资料、设置维修密码等，第三方服务机构很难介入，另一方面，由于没有统一的行业规范标准，第三方服务机构良莠不齐，服务水平差异很大，可能导致各种安全隐患。<h2 style="font-family: Verdana, Arial, Helvetica, 1.8em;text-indent: 2em;">我院地区唯一一家综合性三级甲等医院。目前，医院医疗设备总资产超过5亿元，每年超过一千万元的设备维修费用（2016年我院医疗设备维保费用总额近1200万元）是一项很大的成本支出。医疗设备管理处负责医疗设备的日常管理和维修，设专职维修人员15人，分组协作，维修人员的水平和经验有较大差异。<p style="caret-color: rgb(0, 0, 0);color: rgb(0, 0, 0);font-family: Verdana, Arial, Helvetica, sans-serif;font-size: 1em;white-space: normal;text-size-adjust: auto;margin: 0.5em;line-height: 1.8em;text-indent: 2em;">2016年以前，我院主要依靠医院工程人员自主维修和原厂维修，基本没有第三方介入。自主维修为医院节省了大量维修支出，但由于现代医疗设备的多样性和专业性，受医院维修人员的技术水平、维修资料和配件所限，而且许多临床科室对维修的时限要求很高，设备发生故障时，医院医学工程部门无法及时独立完成维修，只能求助于原厂。<p style="caret-color: rgb(0, 0, 0);color: 1.8em;text-indent: 2em;">① 价格昂贵，一方面个别原厂大型设备的维保服务存在技术（或配件）垄断，价格很高，另一方面原厂维修在判断出故障后，往往直接进行部件的更换，这也拉高了价格；<p style="caret-color: rgb(0, 0, 0);color: rgb(0, 0, 0);font-family: Verdana, Arial, Helvetica, sans-serif;font-size: 1em;white-space: normal;text-size-adjust: auto;margin: 0.5em;line-height: 1.8em;text-indent: 2em;">② 个别原厂只有在收到付款后才启动维修，影响维修进度，导致临床不满。<h2 style="font-family: Verdana, Arial, 0.5em;line-height: 1.8em;text-indent: 2em;">我院选择第三方服务机构遵循以下标准：① 服务质量可靠；② 服务价格合理；③ 信誉良好，业绩优良；④ 备件渠道合法，库存充足；⑤ 响应迅速。<p style="caret-color: rgb(0, 0, 0);color: rgb(0, 0, 0);font-family: Verdana, Arial, 2em;">单次付费维修适用于使用时间不长或故障率低的设备。保修服务要计算好单次维修和保修的效益分析结果，一般来说，保修价格不应高于年平均维修费用的80%。外包服务则适用于医院数量大，故障频发的设备。我院设备维修从2016年开始并逐步扩大与第三方服务机构的合作，合作方式为单次付费维修和保修服务，暂无外包服务方式。合作较多的有大型乙类医疗设备、超声设备、内窥镜设备。<h3 normal;text-size-adjust: auto;margin: 0.5em;line-height: 1.8em;text-indent: 2em;">我院现有大型乙类医疗设备14台，CT机6台，MRI 3台，DSA 3台，SPECT 1台，直线加速器（Linear Accelerator，LA）1台。13台设备总价值占我院医疗设备总价值的25.06%。其配置、使用及维保情况，见表1。<p style="caret-color: rgb(0, 0, 0);color: rgb(0, 0, 0);font-family: Verdana, Arial, Helvetica, sans-serif;font-size: 1em;white-space: 2em;">鉴于乙类大型医用设备的重要地位，为提高设备使用效率，我院为10台乙类设备购买了保修服务合同，保修服务购买率为71.43%，在合作者的选择上，我院4台乙类设备与第三方服务签订了保修合同，占保修和单次付费维修总体的28.57%，占保修服务的40%，另外4台单次付费维修设备则全部采用了原厂维修。表1 我院乙类大型医疗设备配置、使用及维保情况<img class="" 2em;">我院现有超声诊断仪共计36台，总价值占我院医疗设备总价值的9.38%，除去有7台在免保期内和医院工程师自修一些小的故障外，全部采用了第三方服务机构维修。我院为6台彩超购买了保修（其中4台是带病保修），保修服务购买率为16.67%。此外，我院一年内有6台彩超进行过单次维修（其中一台维修过2次），且其中5台的维修费用均低于年保修费用。<h3 style="font-family: 0.5em;line-height: 1.8em;text-indent: 2em;">内窥镜医用设备在医院占有重要地位，我院共有112条（根）内窥镜，其中软性内窥镜有71条，硬性内窥镜41根，分布在多个科室使用，内窥镜总价值占我院医疗设备总价值的5.30%。<p style="caret-color: rgb(0, 0, 0);color: rgb(0, 0, 0);font-family: 2em;">我院设备管理处工程师不能提供内窥镜自主维修服务，全部采用原厂维修或第三方服务维修。我院所有内窥镜均没有购买保修服务（9条软性内窥镜在免保期），一年内共计维修内窥镜33次，共计花费维修费用为88.20万元，占内窥镜总价值的3.3%，其中第三方服务机构维修22次，是原厂维修次数的两倍。<h3 style="font-family: Verdana, Arial, Helvetica, 1.8em;text-indent: 2em;">除大型乙类医疗设备、超声诊断设备和内窥镜设备外，我院在监护类设备、放射类设备、消毒设备、呼吸麻醉设备等也有与第三方合作的案例，但相对较少。<h2 style="font-family: Verdana, Arial, Helvetica, sans-serif;text-indent: 0em;white-space: normal;text-size-adjust: auto;font-size: 1.4em;font-weight: bold;color: rgb(195, 101, 0);text-align: left;margin: 0.7em;">3 我院医疗设备维修与第三方服务机构合作前后对比</h2><p style="caret-color: rgb(0, 0, 0);color: rgb(0, 0, 0);font-family: Verdana, Arial, Helvetica, sans-serif;font-size: 1em;white-space: normal;text-size-adjust: auto;margin: 0.5em;line-height: 1.8em;text-indent: 2em;">我院设备维修与第三方服务机构合作后变化明显，总体来说，利明显大于弊。其好处主要体现在以下四个方面。<p style="caret-color: rgb(0, 0, 0);color: rgb(0, 0, 0);font-family: Verdana, Arial, Helvetica, sans-serif;font-size: 1em;white-space: 2em;">（1）打破了原厂维修价格垄断，降低了设备维保费用支出。2016年9月30日至2017年9月29日一年内，我院全面扩大了与第三方的合作，在设备总资产增加约4000万元的情况下，设备维修保养总费用为1158.62万元，较上一年度（2015年9月30日至2016年9月29日维保总金额1346.74万元，与第三方合作很少）降低了13.97%的维保支出。我院一年内乙类设备、超声诊断仪、内窥镜维保谈判原厂和第三方共同参与最终报价数据，见表2。表2 2em;">（4）较原厂提供备品方便。这尤其反应在超声和内窥镜产品上，超声原厂一般不能提供备品，而第三方可以提供，这也是我院彩超维修自从与第三方合作后没有再用过原厂的原因之一，内窥镜原厂备品大约需要一周的审批周期，第三方不用审批。<p style="caret-color: rgb(0, 0, 0);color: rgb(0, 0, 0);font-family: Verdana, Arial, 2em;">第三方服务机构配件储备基本能够满足维修需要，但因其渠道多样，部分公司监管不严，导致部分配件稳定性不好。我院与某第三方进行维修谈判时，该第三方就曾明确表示，如果允许使用旧设备拆卸的配件可以大幅降低配件价格。建议与第三方服务机构合作时要求使用全新配件，进口配件要求提供报关单，延长相同故障的保修期限。<h2 style="font-family: Verdana, Arial, 1.8em;text-indent: 2em;">医疗机构与第三方服务机构合作有得有失，机遇与风险并存。好处有：<p style="caret-color: rgb(0, 0, 0);color: rgb(0, 0, 0);font-family: Verdana, Arial, Helvetica, sans-serif;font-size: 1em;white-space: normal;text-size-adjust: auto;margin: 0.5em;line-height: 1.8em;text-indent: 2em;">① 打破原厂维修价格垄断，第三方与厂家争夺市场，使价格趋于合理、服务趋于高效化，能降低医院设备维护费用的支出；<p style="caret-color: rgb(0, 0, 0);color: rgb(0, 0,
