超声探头穿刺架的发展历史

从20世纪60年代A型超声应用与穿刺到现在的彩色血流多普勒超声、腔内超声、术中超声、自动弹射活检技术、三维超声、超声造影和导航技术的应用，介入超声已经走过了五十多年的发展，步入了影像与病理、诊断与治疗相结合的新阶段，广泛与应用于临床各科室。  1969年在维也纳第一届世界超声诊断大会上，产科医生Alfred Kratochwi报道了在A型超声引导下用带有中心孔的探头进行羊膜腔穿刺，这成为了介入性超声的开端。 1972年Holm等研制成功了带有中心孔的穿刺探头，并用干腹部脏器的导向穿刺，首次使得在B型声像图中能够同时清晰地显示出病灶和穿刺针尖，并且能预先选择安全的穿刺径路、监视和引导穿刺针准确到达“靶目标”病灶，从根本上改变了传统穿刺方法的盲目性，显著提高了穿刺的安全性和准确性，这标志着临床超声引导穿刺术的开始（见图1）。几乎同时，美国的Goldber也成功研制了带有中心孔的穿刺探头，并开创了超声引导经皮肝穿刺活检，首次实现了经皮将活检器械准确插入肝内病灶取材的夙愿，即进行经皮肝脏活检。  <img data-s="300,640" data-type="png" src="http://mmbiz.qpic.cn/mmbiz_png/dx3KibwtDhvcE2dNZZsDGo0jwFuF60w0c9OYTXrvVsd5IBu3FT6iaApypywQYD07RXdGWibyljH4TPExsRArDWkicg/0?wx_fmt=png" data-ratio="0.8035426731078905" data-w="621" /> 1974年，第一台实时穿刺引导超声仪问世。Pederson等报道超声引导穿刺诊断肾脓肿，并经穿刺针向病灶内注入抗生素，取得了满意的疗效。同时，他们对肾后性尿毒症患者，开展丁(了）超声导向经皮穿刺扩张肾盂和留置导管引流的方法进行治疗。并在同年进行了第一例超声引导下肾造影术。  1977年Pederson等首次报道用附加导向装置的探头进行超声引导穿刺，这种实时穿刺探头由电干线阵探头和侧进式穿刺架组成，提高了穿刺的准确性，对体积小、位置深的病变尤为重要。（见图2）  <img data-s="300,640" data-type="png" src="http://mmbiz.qpic.cn/mmbiz_png/dx3KibwtDhvcE2dNZZsDGo0jwFuF60w0cd7OM7zyPIh4fpKMR3JPBnkKiajRMs8bEibKBiaW4zv7rZPNtZiadqyERJA/0?wx_fmt=png" data-ratio="0.697151424287856" data-w="667" /> 1984年Masahito Saitoh报道了采用Aloka公司扇形探头和附加的穿刺架（见图3）进行肾脏实时穿刺的情况，这成为现今穿刺架的原型。在此基础上，根据不同的临床应用，发展了多种方案的体表探头穿刺引导装置。  <img data-s="300,640" data-type="png" src="http://mmbiz.qpic.cn/mmbiz_png/dx3KibwtDhvcE2dNZZsDGo0jwFuF60w0ccNEZ8QkqPNkGRX3UkLPRmFq5PL7yewwgEmdackq3vlTuH7X0U08GnA/0?wx_fmt=png" data-ratio="0.8498293515358362" data-w="586" /> 1981年，Holm等报道超声导向经会阴部前列腺穿刺活检。将特殊导向装置附于直肠探头上（见图4），进行经会阴穿刺活检，具有高度的准确性，对体积小、位置深的病变穿刺尤为方便。<img data-s="300,640" data-type="png" src="http://mmbiz.qpic.cn/mmbiz_png/dx3KibwtDhvcE2dNZZsDGo0jwFuF60w0cFMubL7OPXB3lKiaH4vGbPsabYdSKb1fibTnIrbiabxtibN68ZfZiawDLo0Q/0?wx_fmt=png" data-ratio="0.7721774193548387" data-w="496" /> 上世纪90年代末，Mary Jane等采用在阴道探头上面安装简易的管状装置（见图5）引导，进行经阴道穿刺、活检和置管引流的临床操作 <img data-s="300,640" data-type="png" src="http://mmbiz.qpic.cn/mmbiz_png/dx3KibwtDhvcE2dNZZsDGo0jwFuF60w0cjpPHBm9GUcDu73JY0R5iaSMr1VDic22ibkNe96bbl5ibGg3uraLyFZstgQ/0?wx_fmt=png" data-ratio="0.24776119402985075" data-w="670" />  2006年12月1日，经上级批准成立了由何文担任主任委员的国内第一个超声介入委员会暨中国医学影像技术研究会超声介入专业委员会，并召开了第一届全国介入性超声学术研讨会，分别在海口、烟台、广州和西安成功举办了四届全国介入性超声学术研讨会，每次参会代表400～500人。2010年中华医学会超声分会又成立了介入学组，同年中国超声医学工程学会也成立丁超声介入委员会。超声介入委员会成立以来一直致力于全面推进全国超声介入技术的发展，在广大医学界同仁的大力支持和参与下，超声介入专业委员团结协作，不断进取，勇于创新，积极开展高水平的学术交流活动，为广大超声医务工作者搭建了很好的学术交流平台，有效地提高了全国介入性超声的水平。促进我国介入性超声的发展，规范介入性超声操作，提高技术水平，加强学术交流。  <span style="max-width: 100%;
