/></span></section><section><span>但报道一出,美国内部便出现反对的声音。美国重症协会、美国呼吸治疗协会、美国麻醉医师协会、麻醉患者安全基金会、美国重症护师协会、美国胸科医师协会等多个学会便紧急发布联合声明,“建议临床医生不要尝试共用呼吸机,使用目前的设备进行这种操作不安全。”</span></section><section><span><br /></p><section><strong><span>“共享呼吸机”</span></strong></section><section><strong><span>可能面临哪些问题?</span></strong></section><section><br /></section><p><span>最大的问题,是无法保证患者生命安全。</span></p><p><span><br /></span></p><section><strong><span>无法独立监测/设置患者参数指标</span></strong></section><p><strong><span>及时准确评估通气情况</span></strong></p><p><span><br /></span></p><p><span>由于呼吸机流量、压力传感器通常设计在吸气/呼气端阀或设备内部,所以,当两名患者同时共用一台呼吸机时(三通管路连接方式,如下图),传感器所监测到的参数指标,比如吸入、呼出潮气量,皆为两患者的总吸入、呼出潮气量。这将导致医生无法判断患者各自的吸入与呼出量,无法判断患者是否符合小潮气量肺保护通气策略,具有肺损伤风险。</span></p><p><span><br PEEP值将无法根据患者各自状况独立监测/调节,所获得的呼吸力学评估,接收的报警监控信息,将失去意义,患者病情改善/恶化也将无法及时发现,患者安全无法保证。</span></section><section><br /></section><section><strong><span>管路连接的不当</span></strong></section><p><strong><span>可能造成进一步的通气障碍</span></strong></p><p><br /></p><section><span>在进行有创或无创通气时,Y型口远离患者的连接方式,会造成很大的死腔样通气(如下图,橙色线条管路即为无效腔)。患者的重复吸入无效腔(死腔)气体,将导致肺泡有效通气减少,影响CO2排除,引起高碳酸血症。即使Y型口靠近患者,死腔增加不明显,但由于连接回路过长,仍可能造成呼气阻力增加。</span></section><section><span><br /></span></section><section><span>除此外,回路容积的增加,可能阻碍触发,引起呼吸机自检无法通过,增加呼吸机使用安全隐患。</span></section><section><span><br /></span></section><p><p><img /></p><section><strong><span>个体肺顺应性的差异</span></strong></section><p><strong><span>将导致潮气量的分布不均</span></strong></p><p><span><br /></span></p><section><span>临床上想要找到两个病情完全一致的患者,几乎是不可能的。即使临床表现接近,两患者肺组织的呼吸力学指标也可能存在较大差异。机械通气时,气体会优先流向肺顺应性好的患者,这可能导致患者肺泡过度膨胀。而肺顺应性较差的一方,则面临通气不足,氧合指标进一步下降的风险。</span></section><section><span><br /></span></section><section><span>也就是说,在没有单向阀的情况下,气体可能在共用管路的患者间发生摆动呼吸,从而导致交叉感染和肺过度扩张。而且,当患者有自主呼吸时,问题将愈加明显。</span></section><section><br /></section><p><span><strong><span>医生可能面临伦理问题</span></strong></span></p><p><span><br /></span></p><section><span>一台呼吸机可以挽救单个患者的生命,但“共享呼吸机”将带来导致所有患者治疗失败的风险。</span></section><section><span><br /></span></section><section><strong><span>纽约长老会 “方案”</span></strong></section><section><strong><span>能否“迎刃而解”?</span></strong></section><section><strong><span><br /></span></strong></section><section><br /></section><p><span> “共享呼吸机”提出后,纽约长老会医院便公布了包括安全风险、所需回路附件、呼吸机设置及自检、初始患者相容性评估、呼吸机参数及报警设置、通气期间患者监测、患者及呼吸机管理、患者脱机策略、相关伦理问题等十五个方面在内的详细“实施方案”。</span></p><p><span><br /></span></p><p><p><img /></section><p><span>似乎声明中提到的可能面临的问题,从“方案”中都能找到答案。比如:</span></p><p><span><br /></span></p><p><strong><span>不能评估每个患者通气情况:</span></strong><span>可通过各自监护仪上呼吸力学监测功能(监测潮气量、压力、流速、PEEP等指标)的使用来掌控。</span></p><p><span><br /></span></p><p><p><img /></p><section><strong><span>不能独立设置每个患者参数:</span></strong><span>从呼吸机功能角度无法解决。但实施“共享呼吸机”的重要前提便是患者相容性的严格评估(重要指标包括驱动压、呼吸频率以及PEEP的差异),只有病情几乎一致的患者,方能匹配共用一台呼吸机。 </span></section><section><br /></section><section><strong><span>不同患者肺顺应性不同导致的气体分布不均:</span></strong><span>容控模式下,气体会优先流向肺顺应性更好的一方患者。但使用压控模式(给予两患者相同的压力),便可解决两患者潮气量分布不均的问题。</span></section><section><br /></section><section><strong><span>患者自主呼吸导致摆动呼吸问题:</span></strong><span>只要患者有自主呼吸,则很难保证“步调一致”、“强弱相同”,摆动呼吸不可避免。建议可使用常规镇静、肌松以阻断自主呼吸。</span></section><section><br /></section><p><strong><span>患者交叉感染问题:</span></strong><span>回路中加装过滤器便可最大限度解决。</span></p><p><br /></p><section><span>但“理想与现实总存在着差距”。<strong>首先</strong>,两患者共用一台呼吸机的严格条件(高度的匹配性),本身便很难实践,且这样的“门槛”将会让很多患者受到限制,偏离了让更多患者接受呼吸治疗的目的。<strong>其次</strong>,即使两患者具有非常高的相容性,使用了这一共享方案,但两名患者共用一套通气参数,是否会影响预后,目前并无相应临床数据加以参考。<strong>再者</strong>,为避免自主呼吸,需长时间使用肌松剂。但对于ARDS患者,肌松剂的使用有严格的适应症和时间限制,必须考虑长时间使用肌松剂对预后的影响。</span></section><section><br /></section><section><span>且针对能否“共享呼吸机”这一问题,有着丰富一线抗疫经验的<strong>杜斌教授、安友仲教授、彭志勇教授在迈瑞“新冠肺炎中外专家对话”中也都明确表示,“除非患者病情完全一致,否则很难实施,会带来很多问题</strong>。” </span></section><p><br
