/></p><p><span>今年共有四千多家美国医院参加评选，在这份榜单上，有癌症、心脏病、神经科、眼科等 16 个专科的排名，并且根据专科排名的情况汇总一份综合排名。<br /></span></p><p><br /></p><p><br /></p><p><span><strong><span>全美医院最新排名结果出炉</span></strong></span></p><p><span><br /></span></p><p><br /></p><p><span>日常生活中，许多人在做决定之前喜欢参照各种各样的排名。<br /></span></p><p><br /></p><p><span>比如这几天高考成绩下来了，考生们选择填报哪所学校，会看看不同机构出版的大学排名。当然，由于选取的考量尺度不一，不同榜单的大学排名顺位有诸多不同，网上也出现了诸如「全国 TOP2 大学有两所，TOP3 大学有五所，TOP10 大学有十所」的段子。</span></p><p><br /></p><p><span>而除了大学排名，老百姓另外一个很看重的排名便是医院排名。究竟医院是如何排名的，什么样的排名才是可靠的排名，这是很多人关心的一个问题。</span></p><p><br /></p><p><span>毕竟，大学排名关乎前程，而医院排名关乎生死。</span></p><p><br /></p><p><span>我们来简单说说排名结果。</span></p><p><br /></p><ul><li><p><span>在综合排名上，位列前三的医院分别是梅奥诊所、克利夫兰诊所和约翰霍普金斯医院。</span></p></li></ul><p><br /></p><ul><li><p><span>在癌症专科排名上，位列前三的医院分别是 MD 安德森癌症中心、纪念斯隆凯特琳癌症中心和梅奥诊所。</span></p></li></ul><p><br 2020-21 综合医院排名榜单</span></section><p><br /></p><p><span>另外，在 U.S. News 的排名中，成人医院和儿童医院排名是分开的。这样做是因为并非每个大医院都有大量接诊儿童肿瘤、儿童肾病等儿童专科疾病的经验，也并非每个大医院都有能力治疗只有几磅重的新生儿、或者因先心病而需要手术的新生儿。</span></p><p><br /></p><p><span>因此，U.S. News 从 2006 年开始独立设置儿童医院排名，儿童医院排名通常会比成人医院榜单提前一个月公布。</span></p><p><br /></p><p><span>今年的儿童医院排名于 6 月 16 日公布，哈佛大学医学院教学附属的波士顿儿童医院位列综合第一，并在儿童神经、儿童肾病等四个专科位列第一。</span></p><p><br /></p><p><span>在美国，至少有 15 项不同机构评选出来的医院排名，但是几乎没有哪一份医院排名的影响力能比 U.S. News 的更大——你只需要看看各大医院的主页就知道了。</span></p><p><br /></p><p><span>例如波士顿儿童医院，一个半世纪以来一直是全球儿科教育、研究和临床的先驱，在许多份医院排行榜中都名列第一，而在波儿的官网上，只放了 U.S. /></p><p><span>当然，大家更关心的肯定是：U.S. News 是怎么给这些医院排名的？为什么它能做到让各大医院都认可？</span></p><p><br /></p><p><span>U.S. News 的最佳专科排名，其实是为那些患有危及生命或罕见疾病的患者设立的，他们需要一家在处理复杂及高风险病例方面表现卓越的医院。</span></p><p><br /></p><p><span>本次排名共有 4554 家美国医院参与评选，U.S. News 对参与评选的医院要求的门槛并不高，并不限定是公立医院或者私立医院，只要这家医院符合下列四个条件中的一个即可参与评选。</span></p><p><br /></p><ul><li><p><span>教学医院</span></p></li></ul><p><br /></p><ul><li><p><span>隶属于某个医学院</span></p></li></ul><p><br /></p><ul><li><p><span>200 张以上床位的</span></p></li></ul><p><br /></p><ul><li><p><span>100 张以上床位，并且拥有 8 种先进技术中的 4 种，如 PET-CT 或精准放疗</span></p></li></ul><p><br /></p><p><span>U.S. News 对 16 个医学专科领域进行了评估和排名，而综合排名取决于专科排名的总成绩。除了 4 个专科（眼科、精神科、康复科和风湿病科）取决于专科医生的问卷调查结果以外，其他专科均以客观数据为依据。</span></p><p><br /></p><p><span>每个医院的客观数据，总共可以分为以下四类。</span></p><p><br /></p><p><strong><span>1、患者的治疗结果（占比 37.5%）</span></strong></p><p><br /></p><p><span>可否成功维持患者的生命是许多患者及家属最为关注的问题，这项指标也是四类指标占比最大的。</span></p><p><span><br /></span></p><p><br /></p><p><span>生存分数通过统计 2016～2018 年间，比较入院 30 天内死亡的住院患者数量数量得来，医院的得分从最低的 1 分到最高的 5 分；</span></p><p><br /></p><p><strong><span>2、患者的经验（占比 5%）</span></strong></p><p><span><br /></span></p><p><br /></p><p><span>这个指标反映了患者的住院总体质量，也包括患者满意度的调查；</span></p><p><br /></p><p><strong><span>3、其他护理相关指标（占比 30%）</span></strong></p><p><br /></p><p><span>这部分包括护士的配备和患者数量等，数据来自美国医院协会 2018 年年度调查；</span></p><p><br /></p><p><strong><span>4、专家推荐（占比 27.5%）</span></strong></p><p><br /></p><p><span>今年，总共有 23000 名医生参与了这项指标的调查，他们需要回答的问题很简单：当你或者家人遇到复杂的疾病时，在不考虑费用和地理位置的情况下，你会求助于哪家医院？</span></p><p><br /></p><p><span>在 U.S. News /></p><p><span>另外，像儿童医院的一些排名还有更加详细的指标。</span></p><p><br /></p><p><span>比如在儿童医院心脏专科排名这一项，评价各个参选医院的指标非常详细，仅患者治疗结果和经验部分，就包括先心病手术术后存活率、心脏病手术术后并发症概率、心脏移植后存活率、术后平均住院时间、院感防控能力、压疮防控能力等多个指标。</span></p><p><br /></p><p><span><strong><span>我们需要怎样的医院排名？</span></strong></span></p><p><br /></p><p><span>那么，什么人会看医院排名呢？医院管理者会看，医护人员会看，准备找工作的医学生会看，保险公司也会看。</span></p><p><br /></p><p><span>但是最需要这份排名的人，是广大的患者。</span></p><p><span><br /></span></p><p><br /></p><p><span>对于患者而言，什么医院对我的病更有经验、治疗效果更好，这是医院排名需要解决的核心问题。我说句更加直接点的，如果一份医院榜单无法给患者提供这方面的有效参考，那么它跟废纸的区别不大。</span></p><p><span><br /></span></p><p><br /></p><p><span>一些榜单纯粹基于科研水平、SCI 发表数量给医院进行排名，在某种意义上算不算是一种迷惑行为？</span></p><p><span><br /></span></p><p><br /></p><p><span>思考一下，咱们作为医生，如果你自己的家人生病了，你是愿意给他找一位论文发表特别多、但是临床经验不确定的医生，还是找一位科研表现平平、但是临床水平非常高的医生？</span></p><p><br /></p><p><strong><span>我想绝大多数人的选择都是一致的。</span></strong></p><p><br /></p><p><span>医学应当以人为本，医院排名也应当以患者为本，相关数据应当是患者最关心的。但如果想做到这一点，就得涉及到各家医院的深水区了——数据公开。</span></p><p><br /></p><p><span>我们举个例子，我们想给一个国家的癌症医院进行排名。</span></p><p><br /></p><ul><li><p><span>青铜榜单：根据各家医院的床位多 / SCI 文章数量排名。</span></p></li></ul><p><br /></p><ul><li><p><span>黄金榜单：根据各家医院患者数量 / 患者生存率 / 患者安全得分进行排名。</span></p></li></ul><p><br /></p><ul><li><p><span>王者榜单：根据各家医院患者数量 / 不同类型癌症手术数量 / 不同癌症分期五年生存率进行排名。</span></p></li></ul><p><br /></p><p><span>如果这个国家有很多医院数据统计做的非常糟糕，或者虽然有数据但是不愿意公开，那么任何一家医院排名机构都无法做到准确评估。</span></p><p><br /></p><p><span>在这方面，除了 U.S. News 的医院排名，日本的医院排名也值得借鉴。</span></p><p><br /></p><p><span>假设是一名患有脑动脉瘤的男士，他想知道哪家医院对他的病更有经验，那么他可以看这本书，因为这本书将日本各个地区不同医院过去一年治疗脑动脉瘤的数量做了统计，甚至还区分是开颅手术还是血管介入手术、是已破裂还是未破裂的脑动脉瘤。</span></p><p><span><br /></span></p><p><br /></p><p><p><img /></p><p><span>假设是一名刚确诊早期胃癌的女士，她想知道哪家医院做这个手术效果更好，那么她可以看这本书，因为这本书详细统计了各大医院胃癌的手术数量及五年生存率，甚至细致到各个分期（I 期～IV 期）的五年生存率。</span></p><p><span><br /></span></p><p><br /></p><p><p><img 版」这样的医院排名书多年来在日本一直很畅销，因为它为老百姓真真切切地提供了可靠的就医建议。</span></p><p><br /></p><p><span>在日本详细排名的背后，离不开医院的数据公开。如果朝日新闻无法获得这些数据的话，上面这些排名就成为泡影了。</span></p><p><br /></p><p><span>当然，并非所有的患者都会依赖这些医院排名，由 Robert Wood Johnson 基金会和哈佛大学公卫学院联合进行的一项调查发现，受访者中有 38% 的人说他们会根据排名来选择医院，而有 57% 的人说他们更愿意去自己熟悉的医院，而不是去排名靠前的医院。</span></p><p><br /></p><p><span>不过，看不看排名是一回事，有没有可靠的排名又是另外一回事。</span></p><p><br
